The duration of medical and dental education varies considerably, and the curriculum within a discipline varies from country to country. In North America, it takes about ten years (including premed, med-school, and residency) to earn the medical degree needed to practice. The Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree is conferred after satisfactory completion of the course of study in an accredited medical school. Some students opt for a dual degree research-focused MD/PhD program, which typically lasts 7–10 years. The first-year training after earning the MD or DO degree is commonly called an internship, before the residency training as a specialist called the “fellowship.” In some states, the fellowship can begin immediately after the MD/DO degree. In contrast, other states require generalist (unstreamed) training for some years before starting the fellowship program (Scarborough, Turner, and Gregg, n.d.).

The science of medicine is vast as an open sky. There exist different types of medical practices all around the world, which are completely scientific. It is not possible for a single person to be a master of medicine. Similar to the conventional physicians with the Master of Medicine (MD) degree are specialized in a single form of medicine known as Allopathy, the physicians who have qualified in an equivalent MD degree and received a practice license from an accredited medical school on Naturopathy, uses “ND” after their names. Figure 11.1 gives an overview of different forms of medical practices that are practiced worldwide in a hierarchical manner.

The MMD-HP (the most recent revision of the MDS) serves to explore individual, unit, and system level causes of MD. These results provide guidance on how health systems may begin to address MD. To suggest that none of the measures inform a targeted intervention plan fails to demonstrate that empirical research on MD shows significant alignment with the National Academies of Medicine’s National Plan for Health Workforce Well-Being (NAM 2022). For example, research has demonstrated that psychological safety and ethical workplace cultures are interventions aimed to reduce MD. Fostering healthy, safe, and protective work environments includes the presence of supportive managers and supervisors, the collaboration and transparency across team members, and organizational/leadership appreciation, respect, and investment in the health and well-being of the health care workforce (Thomas et al. 2021).

Within the USA, core medical education content requirements are primarily dictated by the Liaison Committee on Medical Education (LCME) and first year medical students are expected to enter training with a baseline of knowledge and competencies.13 Students in the USA enter medical school after completing a Bachelor’s degree and required pre-medical school classes that include the basic sciences.14 Although much is changing in medical education, medical students are typically expected to complete 18–24 months of pre-clinical training (M1 and M2 years) prior to completing 24–30 months of clinical rotations (M3 and M4 years). After completing these 4-years in Undergraduate Medical Education (UME) students earn a Doctor of Medicine (MD) and enter residency training in their chosen specialty. Although each medical school has the flexibility to establish its own curriculum, this is done within the framework of general competencies required for LCME accreditation.15

In the USA, medical students seeking a Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) diploma and further graduate training must successfully complete national licensing exams. Data collected by the National Resident Matching Program from graduate residency programs cites performance on these licensing exams (National Board of Medical Examiners [NBME] USMLE Step 1; National Board of Osteopathic Medical Examiners [NBOME] COMLEX-USA Level 1; NBME USMLE Step 2; and NBOME COMLEX-USA Level 2) as the first and fourth most considered factors when selecting medical students for consideration for admission to the graduate program (National Resident Matching Program 2018). The same survey cites clinical specialty course (clerkship) grades as the sixth most important factor for consideration for medical student selection into graduate residency program (2018). Often, one component of clinical clerkship grades is performance on a standardized national subject exam (the NBME Subject Exam (‘shelf exams’) or NBOME Comprehensive Osteopathic Medical Achievement Test (COMAT)) (National Resident Matching Program 2018).
