Syndromes of insulin resistance are rare. They include genetic defects in the insulin receptor (Donohue syndrome/leprechaunism; Rabson–Mendenhall syndrome; type A insulin resistance) and downstream insulin signalling defects. Acquired insulin resistance (type B) may be caused by antibodies against the insulin receptor.

This meta-analysis was conducted in accordance with the guidelines of PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analysis). A systematic literature search was conducted between January 2014 and January 2019 in the following databases including Ovid-Medline, Science Web, Scopus, Cochrane, Embase, ProQuest. To identify more appropriate research, reference lists of all chosen papers were manually obtained. Due to increase of sensitivity and specificity, a combination of the following search Medical Subject Headings (MESH) and non-MESH terms has been used to detect studies: “Crocus” OR “Saffron” OR “Saffrons” OR “Crocus sativus” OR “Saffron Crocus” OR “Crocus, Saffron” OR “Crocin” AND “Metabolic Syndromes” OR “Metabolic Syndrome X” OR “Insulin Resistance Syndrome X” OR “Metabolic X Syndrome” OR “Metabolic Cardiovascular Syndrome” OR “Dysmetabolic Syndrome X” OR “Blood Sugar” OR “Sugar, Blood” OR “Glucose, Blood” OR “Blood Glucose” OR “Diabetes Mellitus” OR “Diabetes Insipidus” OR “Diet, Diabetic” OR “Prediabetic State” OR “Scleredema Adultorum” OR “Glycation End Products, Advanced” OR “Glucose Intolerance” OR “Gastroparesis” OR “Diabetes Mellitus, Type 1” OR “Wolfram Syndrome” OR “Diabetes Mellitus, Type 2” OR “Diabetes Mellitus, Lipoatrophic” OR “Diabetes, Gestational” OR “Diabetic Ketoacidosis” OR “Donohue Syndrome” OR “Latent Autoimmune Diabetes in Adults” OR “Prediabetic State”OR “Diabete*” OR “Glycem*” and “randomised controlled trial” OR “randomised controlled trials were selected as keywords.
