The transoral incisionless fundoplication (TIF) with EsophyX (EndoGastric Solutions) is heralded as the endoluminal alternative to conventional fundoplication. The procedure involves reduction of any hiatus hernia and creation of a partial gastric wrap, using endoscopically delivered serosa–serosa fasteners. It has been shown to reduce symptoms and medication use, but again in paediatric practice its use is limited because of the large size of the endoscopic mechanism.

Management of GERD consists of lifestyle modifications, pharmacological therapy, anti-reflux surgery, and endoscopic treatment [4]. Proton-pump inhibitors (PPIs) are the drugs of choice in GERD. Despite the administration of PPIs, more than half of patients had persistent GERD symptoms, which was called PPI-refractory GERD [5]. Nissen fundoplication was superior to medical therapy for patients with regurgitation refractory to PPI therapy [6], but post-fundoplication complications were common [7]. With the development of endoscopic technology, endoscopic treatment of refractory GERD was emerging, such as anti-reflux mucosectomy (ARMS), endoscopic radiofrequency anti-reflux therapy (Stretta), and transoral incisionless fundoplication (TIF) [8–10]. Peroral endoscopic cardial constriction (PECC) can narrow the cardia through mucosal ligation to alleviate acid reflux symptoms [11]. Although there were two studies evaluating the improvement of reflux-related symptoms after PECC, researches on the improvement of health-related quality of life are lacking [12,13].

Transoral incisionless fundoplication using the EsophyX device (Redmond, Washington, USA) is an endoscopically mediated mechanical repair of a defective gastroesophageal valve; the main complications are fistulization, difficult belching, peritonitis, shoulder pain and dysphagia [29], when compared to the endoscopic band ligation, this intervention is still invasive, needs more expertise and less cost-effective.
