Wisdom Teeth: Taking bromelain by mouth during the perioperative period for surgical removal of wisdom teeth reduced complications after third molar extraction, including pain, swelling, and limited mouth opening.25, 26, 27 Patients were given oral bromelain enteric-coated capsules (30,000 units) on the day of the surgery, three times/day and continued for three days. Clove oil contains eugenol which acts as an anesthetic and antibacterial agent. Applied by cotton swab or cotton ball over the gums, clove oil may be as effective as benzocaine, the numbing ingredient in over-the-counter toothache gels.28

Salvadora persica has astringent, analgesic and anti-inflammatory activities, which makes it an efficient treatment for primary inflammatory disorders of gums. Moreover, it has been observed that people who were using miswak daily had a decreased toothache incidence contrasted with the users of the conventional toothbrush (Niazi et al., 2016).

Globally, millions of people suffer from toothache due to tooth cavities and often permanent tooth loss. Dental caries, also known as tooth decay is a biofilm-dependent infectious disease that damages teeth by loss of minerals and presents high incidence around restorative polymeric fillings (tooth-colored fillings) (Askar et al. 2020). Untreated caries results in severe pulpal pathologies, which proceeds to tooth loss because the dental enamel cannot regenerate. Also, dental caries is highly prevalent among the elderly population due to unhealthy dietary habits and poor oral hygiene. According to the World Dental Federation, approximately 3.9 billion individuals are affected by dental caries annually, which affects almost half of the world’s population (Martins et al. 2017; Edelstein 2006).
Implementation of oral hygiene practices in nursing homes – the view of supervisor nurses

Our results are parallel with previous studies which have reported that 90% of nurses brush their teeth twice a day and at least 40% of them floss once a day [24] and that 30% of nurses attended the dentist only when they have a toothache or some other dental problems [25]. It has to be noted, however, that supervisor nurses may have reported too high tooth brushing frequency when compared to females with the same age groups in a national health survey [26]. In our study, supervisor nurses’ own oral health habits were associated with better “Organizational practices.” It has been found that improving dental knowledge among nurses is successful in improving oral health among older people [9]. Most of the nurses in our study reported that they want to get more oral health education which is in line with previous studies which have reported that nurses are willing to receive more education about oral health care [13,27]. Oral health education for supervisor nurses would improve their own oral health habits as well as the implementation of oral health-related practices in their care units.
Fomepizole as an adjunct in acetylcysteine treated acetaminophen overdose patients: a case series

A 47-year-old woman presented to a community hospital with “lightheadedness” and confusion. She had hypoglycemia with a blood glucose of 49 mg/dL by finger stick and significantly elevated serum lactate (12.1 mmol/L). There was a delay in obtaining the collateral information about her ingesting 1 gram of acetaminophen every 4 h for 2 to 3 days for a toothache. Initial APAP level of 10.8 mcg/mL was an estimated 16 h post last dose. In this case the APAP*AT level was significantly elevated with a value of 26322. IV NAC was administered at 150 mg/kg loading dose over 1 h followed by 12.5 mg/kg/hour. The patient was transferred to a tertiary care, academic hospital ICU for further management. She presented critically ill and within a few hours was intubated on mechanical ventilation. She was hypotensive and required three vasopressors (norepinephrine, vasopressin, and Angiotensin II). She had AKI with elevating creatinine and was started on was started on CRRT. IV NAC was continued from the referring hospital. At hour 24 she received a stat dose of fomepizole 15 mg/kg over 30 min and a repeat dose 4 h after CRRT. Her transaminases continued to rise, and she was given an additional dose of fomepizole at hour 57 followed by a final dose at hour 70. She made a full recovery and did not have any signs of liver failure at discharge.

Non-steroidal anti-inflammatory drug (NSAID) is a kind of drugs which were widely used in therapy to alleviate pain and fever. Ibuprofen, a well-known one of NSAIDs, has been used in alleviating most of moderate pain, such as headache, toothache, and joint pain for several decades (Ipek et al. 2019). Due to the poor solubility and the low dissolution rate in water of ibuprofen (Ren et al. 2017), it was often prepared into solid dispersion in order to improve its dissolution property (Shi et al. 2018). The amorphous state of ibuprofen in solid dispersion is the main reason for its improved dissolution property, because of that substance in amorphous state has the lowest energy to dissolved into solvent compared with its other crystal state. However, the stability of the amorphous state is usually poor. Amorphous state is usually tended to transform into another crystal state. When it happens in storage period, it often leads to undesirable changes in dissolution property and influences the preparation’s qualities.
