Steatocystoma multiplex can be a distressing condition characterized by multiple, asymptomatic, dermal cysts that usually occur on the trunk, face, and proximal extremities (Figure 8.36). The inner contents of these cysts appear like thick white toothpaste rather than the keratinaceous content of epidermal inclusion cysts (Figure 8.37). These dermal cysts may become infected and painful, and produce a visible exudate. Surgical drainage is often used but there has been some success with both oral retinoids and cryosurgery. In one case report a 10-second nitrogen spray was applied to non-inflamed lesions that were smaller than 2 cm. Six months after treatment, the cysts were flattened, leaving some hypopigmentation.33 We recommend 10 s of cryosurgery with an open-spray technique as an acceptable alternative to surgery.

Other conditions for which it has been suggested that isotretinoin is beneficial, include: Vohwinkel syndrome with ichthyosis (10), lichen amyloidosis (13), lymphangioma circumscriptum (16), erythema dyschromicum (6), squamous cell carcinoma on epidermoid cyst (17), unresectable perineal cuniculatum carcinoma (18), chronic granulomatous disease (24), erythrokeratoderma variablilis (25), atrophoderma vermiculatum (26,39), synovitis/hyperostosis/osteitis syndrome and acne (27), prurigo pigmentosa (31), vesiculous prurigo (32), follicular mucinosis (33), granuloma annulare (36), steatocystoma multiplex suppurativum (37), and nipple hyperkeratosis (38). Case studies have also been published which show evidence for benefit of isotretinoin in lichen planus (49), psoriasis (30,51,53), cutaneous lupus (21), and eruptive histocytoma (28). Most reports declared that although isotretinoin had proved helpful, the authors were not sure on its mechanism of action in those conditions. Those that did, felt isotretinoin’s mechanism of action were: anti-inflammatory (49), anti-angiogenic (16), immunomodulatory (6), sebosuppressive (33), and anti-neoplastic (43).

Steatocystoma multiplex often occurs around the area of the chest or axilla, and sometimes is a cause for concern as it causes disfiguring features. Recurrent inflammation in steatocystomas may irritate the patient to attempt squeezing the contents out of the skin to no avail. The most effective course of action would be through surgical intervention. However, under the circumstances where multiple steatocystomas are present, numerous surgical scars may not be acceptable.
