Pentoxifylline is a xanthine derivative that is mainly used for platelet inhibition and improvement of microcirculation. The drug also displays immunomodulatory effects that are probably mediated via inhibition of TNF-α and interleukin-1. Combined therapy with pentoxifylline and tocopherol has been used successfully in the treatment of radiation-induced fibrosis of the skin and subcutaneous tissue,60 and there are animal data suggesting pentoxifylline-mediated inhibition of bleomycin-induced lung injury. There is limited experience showing a beneficial effect of pentoxifylline in preventing the development of radiation-induced lung injury in patients irradiated for breast and lung cancer,61 but this needs to be confirmed in a large prospective trial.

Pentoxifylline improves RBC deformability and is associated with a reduction in viscosity [70]. Limited experience in the context of LVAD thrombosis and hemolysis suggests a possible role for pentoxifylline [71]. A double-blinded randomized controlled trial in patients who undergo an axillary 5.0 or 5.5 pVAD insertion for acute decompensated heart failure is planned to study this treatment strategy (NCT04391231).
