The capsule is incised in a V shape, based superiorly. The first incision is immediately adjacent to the edge of the rectus medially from above the neck to below it. The reflected head of the rectus may be divided to improve visualisation, but this is not generally necessary. It is important to take this capsule incision distally beyond the neck as this will allow easier removal of the femoral head later. The second limb of the V incision is along the intertrochanteric line of the femur. This is done more easily after placing a suture at the apex of the V for retraction, which can also be used later to close the capsule. I find it easiest to make this part of the capsulotomy with a diathermy, passing from inside the capsule to outside. The capsule incision is continued proximally to the junction of the neck and shaft. The two retractors are then moved so that they each lie within the capsule, to allow excellent visualisation of the neck (Fig. 17.8). The lesser trochanter (LT) is still not visible, but the intertrochanteric line of the femur is visible and is known to run to the superior edge of the LT.

Here are the coxofemoral articulations: Iliofemoral ligament is the most powerful ligament of the coxofemoral joint. This ligament has the shape of a fan and is very large. This ligament can support a 350–500 kilogram mass. Insertion: The origin is on anteroinferior iliac spine. The distal insertion is on the intertrochanteric line of the anteriomedial surface of the femur. This line runs obliquely from the greater trochanter to the lesser trochanter.

The intertrochanteric line runs from the greater trochanter to the lesser trochanter anteriorly in the proximal end of the femur. It curves medially below the lesser trochanter and runs around the femoral shaft and merges with its medial margin to continue as the spiral line (Joint Structure and Function : Joint Structure and Function , 2005).
