Inferior rectus

The inferior rectus is a muscle in the eye that is responsible for moving the eye downward, rotating the top of the eye away from the nose, and moving the eye inward. The most common type of orbital fracture is a blowout-type fracture of the orbital floor. Blowout orbital fracture is usually the result of a direct blow with sudden increase of intraorbital pressure producing a fracture of the orbital floor. This fracture may result in downward herniation of orbital contents into the maxillary sinus. Diplopia may occur due to entrapment of the inferior rectus muscle in the fracture. Radiographs may show a soft tissue mass in the shape of a ‘teardrop’ in the roof of the maxillary sinus (Fig. 11.3) due to downward herniation of orbital fat. The actual fracture is usually quite difficult to see radiographically. Coronal plane CT shows the fracture, as well as herniation of orbital structures into the maxillary sinus (Fig. 11.4).

A 64 years old female patient presented to our department with facial asymmetry due to an extensive zygoma and orbital bone defect caused by a traumatic event that had occurred 6 months before. (Figure 1). She did not have any prior surgical treatment and presented diplopia (double vision), epiphora (overflow of tears onto the face) and a left infraorbital scar. At the initial consult, she had notable enophthalmia (eye that is abnormally sunken into the orbit), dystopia (malposition) of the left eye, movement impairment of left eye upward gaze, caused by the entrapment of the inferior rectus muscle.
