We believe that the domination and control of the victim is what is eroticized and gratifying sexually. Genital contact or ejaculation is not necessary for sexual satisfaction, a conclusion widely held by authorities on sexual behavior for quite some time (e.g., Freud, 1905; Socarides, 1988).

Erection and ejaculation during sexual intercourse are closely associated but are independently regulated [45]. Ejaculation represents the climax of the sexual cycle and comprises of emission (secretion of semen) and expulsion (propulsion of semen) phases. Various DAR are expressed in tissues that participate in ejaculation, including vas deferens [46], prostate [47], and seminal vesicles [40]. Although DA has been considered an important factor in the control of ejaculation [22], given the necessity to study ejaculation in live animals, which cannot be replicated in vitro, it has been difficult to specifically assign central vs. peripheral effects of DA on this process.

In experimental animals rectal stimulation can also be given for ejaculation. It is extremely difficult to insert the electrode without anesthetizing the animal especially in case of monkeys. Anesthesia raises the threshold of stimulation and inadvertently rectal mucosal burns may occur. Also, the ejaculate obtained by rectal stimulation solidifies rapidly probably because of higher contents of secretions from the accessory glands. Liquefaction for the study of spermatozoal motility takes time with likely change in spermatozoal function. For these reasons in our laboratory at the All India Institute of Medical Sciences, New Delhi, we have preferred to use penile stimulation in studies on the rhesus monkey. Two aluminium foil electrodes 7mm wide moistened with saline are wrapped around the penis. One electrode is positioned just posterior to the glans penis and the other near the penile base bordering the peno-scrotal junction (Figure 12.2). Rectangular pulses of duration 100 μs and 20-Hz frequency are delivered to the electrodes. Ejaculatory response is best if the amplitude is rapidly increased starting from a zero level. More often than not, ejaculation is obtained without erection. With monkeys there is a human operator-animal interactive component which has strong bias on the outcome of the procedure. Some monkeys seem to prefer specific individuals and produce an ejaculate readily when handled by them. Also a reward component is important. Clearly even in electroejaculation higher neural centers have modifying influences.

A total of 912 participants were randomized from the 13 included studies: 472 to the intervention group and 440 to the control group. Seventy-nine participants dropped out during the studies and were excluded from statistical analysis. The age of participants ranged from 19 to 83 years. The underlying causes for participants identifying as having erectile dysfunction included radical prostatectomy, myocardial infarction, hypertension, ischemic heart disease, venous occlusion, arterial insufficiency, and psychogenic causes. Participants with premature ejaculation were identified as having lifelong premature ejaculation. The severity of erectile dysfunction in all the studies was mild to severe. Erectile dysfunction had lasted between 6 and 360 months in most participants in the studies.

Due to the fact that SSRIs delay ejaculation in men, they are successfully used to treat premature ejaculation (PE) (Waldinger et al. 2004), with dapoxetine being particularly effective (McMahon 2011). Delaying ejaculation in patients with PE improves the quality of sexual intercourse and increases coital frequency, so that SSRIs can positively affect fertility. On the other hand, excessive delay in ejaculation can also indirectly increase the risk of infertility (Kondoh 2012). Therefore, the use of SSRIs in patients with initially normal ejaculatory function may reduce the likelihood of conception if delayed ejaculation occurs as an adverse effect. Other sexual side effects of SSRIs, such as decreased libido and erectile dysfunction, can also have negative consequences for conception (Berger et al. 2016).

Not all individuals will have equal opportunities for pleasurable sexual experiences (Hall, 2019). More specifically, experiences of sexual pleasure seem embedded in a gendered3 context. Around the world, sexual complaints are more prevalent in heterosexual women than in heterosexual men (Laumann et al., 2005). In most contemporary societies, heterosexual women’s sexual pleasure is still generally subordinated to heterosexual men’s pleasure (Hall, 2019; van Lunsen et al., 2013). In (religious) traditional cultures sexual pleasure of women is judged to be more dangerous and undesirable than that of men, or even considered irrelevant (Hall, 2019), probably because sexual pleasure of women does not appear to be directly related to reproductive success. Sexual pleasure of men, at least pleasure entailing orgasm with ejaculation, is condoned, if not prioritized, as procreational duties depend on it. Consequently, only those problems that interfere with sexual pleasure of men or with procreative sex are deemed worthy of complaint or treatment (Hall, 2019), while sexual issues that are important and distressing to women (and individuals engaging in non-procreative sex), including a lack of sexual pleasure, go unnoticed.
