One element of the Build vs. Buy Decision that is somewhat specific to DHA developers is the special role of privacy and security in the industry. Outsourcing the development of certain components can have significant legal or reputational implications for the DHA developer. For example, the developer of Flo Period & Ovulation Tracker among others were criticized in the press for sharing personal health data with Facebook, the developer of the analytics Software Development Kit (SDK) the app was using.10 Another series of articles sharply criticized various apps for sharing information with large tech companies that are also prominent cloud vendors.11 From these situations and countless others, it's clear that outsourcing decisions, including the selection of the specific outsourcing vendor, can be make or break decisions, particularly when patient data processing is involved. In all situations where vendors are used, their use and role should be clearly described in user privacy policies.

High-Flow Microcatheters: These catheters are able to be power injected. However, a significant amount of pressure can be transmitted to the vessel, and careful consideration and adjustment of pounds per square inch (psi) must be taken to avoid vessel injury. Commonly used examples are the 2.8Fr Progreat (Terumo) or Renegade Hi-Flo (Boston Scientific). Renegade Hi-Flo comes in different variants such as the Fathom, Transend, and STC. These are braided hydrophilic catheters with platinum or steel, which allows for flexibility.Progreat (Terumo): These hydrophilic catheters are made out of Teflon with a Tungsten coil.

Staff also need to be trained not to let information slip unintentionally. Most realize that it would be wrong to tell Auntie Flo’ the result of her niece’s pregnancy test, but few understand that to confirm to a husband that his wife has an appointment might be a technical breach of confidence. Although a slip of this sort seldom matters, it is best to have strict rules regarding the release of information to anyone other than the patient concerned. All staff should be encouraged to develop total amnesia about what has gone on in the practice as soon as they leave work!
Prevalence, content and significance of advance care planning in nursing home patients

Although many elderly people wish to participate in advance care planning [1,3,8–12], EOL discussions, which are crucial elements of care planning, are reported to be rare [1,8–10,13–17]. Sharp et al. showed that only 2–29% had discussed EOL care plans with healthcare staff [10]. In an Australian study, only 0.2% of nursing home patients had a care plan [13]. The practice of care planning is well established in Sweden. Flo et al. concluded in a review that studies on advance care planning in nursing homes are rare and that there are variations in the definitions and content of advance care planning [18]. In the various definitions of advance care planning, a decision-making process is often part of the content [19–21]. Some also highlight the aspect of preparing the patient and family members for EOL [19–21]. Aspects frequently present in advance care planning are proactive planning for future care, patient preferences concerning treatment and care, and involvement of family members. These aspects are central in the EAPC (European Association of Palliative Care)-supported consensus-based definition of advance care planning [19]. In Sweden, a decision to shift focus from any level of life-prolonging or life-sustaining care to strictly palliative care is a widely used strategy, referred to as ‘breakpoint decision’ [22] and equals a palliative care plan.

The current study was designed to address the above objectives with the utilization of the validated PROs, the FLO-11 questionnaire and the FLSQ (23). Additionally, the validated FWS-A was employed to complement these PROs and was administered by the physician investigator, but also independently scored by the subject. When these scales were administered at the scheduled follow-up visits, a clinically meaningful improvement in GL was documented at 30 days after a single treatment of onabotulinumtoxinA. Importantly, this treatment outcome was maintained in the majority of subjects for the duration of the 4-month study (Figures 1–3). Notably, in the cohort of subjects who had moderate to severe GL at rest prior to treatment, more than 75% of subjects achieved a clinically relevant improvement in their GL at day 30, as independently assessed by the physicians and the subjects, with this outcome also maintained for the duration of the study (Figure 7).

Among the input variables, X1 was considered as the most significant factor for Y1. As shown in Table 2, all preparations had nano-dispersions, indicating that ATV was sufficiently surrounded by surfactants. Because ATV is a hydrophobic drug with poor solubility in water, the hydrophobic chain structure of the surfactant component was important to sufficiently surround it with a surfactant. In combination with the equations, as illustrated in Figure 4(A), a sharp change in Y1 was observed when X1 shifted from the lower limit (-1) to the upper limit (1). Surfactants with shorter hydrophobic chains give a positive curvature effect to the emulsion, when compared to surfactants with longer aliphatic chains. G48 has a longer hydrophobic chain including palmitate (C16) and stearate (C18) than that of T20 (laureate, C12). Therefore, T20 formed smaller micelles than G48 (Oliver et al. 2013). In addition, X3 had a statistically negative relationship with Y1. As a water-insoluble carrier, FLO has a mesoporous structure with a small particle size and a large surface area, resulting in a high oil absorption capacity (Zhou et al. 2017). On the other hand, VP105 is water-soluble and easily dispersed by releasing the micelle component from the solid carrier (Yeom et al. 2017a). A negative correlation was established between X3 and Y1, which meant that as the ratio of VP105 increases, the droplet size decreases. This may be due to the presence of water-insoluble solid carriers such as FLO and/or incomplete desorption of micelle components (Kang et al. 2011; Mandić et al. 2017). In other words, as the ratio of VP105, a water-soluble solid carrier, increases and the ratio of FLO decreases, desorption of the adsorbed micelle component occurred more easily. Accordingly, evenly distributed smaller micelles are properly constructed.
