A number of barrier-analysis tools already exist. The tool below is my adaptation of existing models. If we start with the risks identified from learning from our RCA, then the solutions may emerge from consideration of improvements for existing barriers or from the creation of new barriers. I stress that solutions may arise as there may be occasions where no new barriers are either evident or realistic. We are looking for achievable within the service in question. A potential barrier that involves work or investment beyond the scope or expertise of the investigation team may be ‘exported’ or referred to other appropriate authorities.
Epithelial barrier dysfunction and cell migration induction via JNK/cofilin/actin by angubindin-1

To investigate the effects of angubindin-1 on the epithelial barrier and proteins of tTJ and signaling, cells were treated with 2.5 or 5 μg/ml angubindin-1 and subjected to epithelial barrier analysis, immunocytochemical analysis for LSR and tricellulin, and western blot analysis for LSR, tricellulin, pYAP, YAP, pJNK and JNK. The values of TEER were decreased by treatment with angubindin-1 in a dose-dependent manner (Figure 1(a)). Western blot analysis revealed that the treatment with 2.5 μg/ml angubindin-1 decreased LSR expression and increased the expression of pYAP, YAP and pJNK (Figure 1(b)). Immunocytochemical analysis showed that the treatment with 2.5 μg/ml angubindin-1 decreased LSR expression at the membranes of tricellular contacts (Figure 1(c)). When angubindin-1 was removed from the medium after the treatment with it at 2.5 μg/ml for 48 h, the values of TEER and LSR expression recovered until 24 h after the removal (Figure 1(d,e)).

In recent years, many applications of fuzzy set theory have been used as an extension to FTA. Nadjafi et al. [14] discussed that the limitations of traditional FTA, including difficulty to compute the probability of events in multi-state systems, are a result of the fact that it is time-consuming and does not provide adequate or known information for the components. Therefore, they used Monte Carlo simulation for this purpose in the fuzzy environment to deal with any ambiguities that may happen during the study. The authors applied the proposed approach to a launch emergency detection system as a multi-state system. This approach was further developed for the most generic case where the components have multi-states. Guan et al. [15] applied a risk assessment employing traditional FTA to a ship diesel room for fire and explosion accidents. They tried to determine the critical component which has a high probability of triggering an accident. Therefore, the minimal cut set as a measurement tool was used throughout the analysis. Accordingly, the safety performance of specified field was improved with respect to measurement results obtained for Chinese inland dual-fuel ships. Ramzali et al. [16] engaged fuzzy triangular and fuzzy trapezoidal sets to cope with uncertainties in the safety barrier analysis of an offshore drilling system. A reliability block diagram and FTA were employed to improve the safety barrier performance. Using both fuzzy sets shows that the proposed approach was trustable to reduce the uncertainties of expert opinion for computing each event. Additionally, the same approach was used for consequent assessment of event tree analysis (ETA).
Phone CPR and barriers affecting life-saving seconds

During the one-year period, 244 phone CPR calls were selected based on the word filters mentioned above. From these, in total 121 were discarded based on the exclusion criteria. The remaining 123 phone calls were included for barrier analysis. Of these, 52 calls were excluded for evaluation of time analysis since phone CPR was not started (Figure 1).
