Methisazone is a thiosemicarbazone antiviral drug that is effective in the treatment of poxviruses. It is only about 40 percent effective against smallpox. The drug was given at several times the human dose to pregnant rabbits and mice, and no increased frequency of birth defects was shown (Grosenbach et al., 2011).

These compounds have been described as prophylactic agents against smallpox [56]. Isatin derivatives include methisazone and β-thiosemicarbazone. However, their direct therapeutic efficacy has not been proven. Both compounds were evaluated in mice models challenged with the vaccinia virus or cowpox virus [57]. The compounds were administered intraperitoneally at 3, 10, and 30 mg/kg for five days beginning 24, 48, or 72 h post-infection. Strikingly, both compounds were effective in surviving mice infected with the vaccinia virus, but no efficacy was found in the group of cowpox virus infection. Methisazone (marboran) is believed to be an inhibitor of the viral RNA polymerase; however, its mechanism for transcription inhibition is still unclear owing to its limited clinical experience in humans.

Methisazone by inhibiting protein synthesis and ABT450 by inhibiting nsps have exhibited their respective antiviral potential [154]. Moreover, exaggerated inflammation, cytokine storm and inadequate immune response are some characteristic features of COVID-19 infection [154]. Zhang et al. exhibited melatonin as an important therapeutic approach against cytokine storm owing to its ability to act on one or several intermediate steps in the process and thereby demonstrated potential advantages of melatonin in the management of SARS-CoV2.
