Usually Burkholderia cepacia is moderately resistant to cefoperazone. Cefoperazone is similar to other beta-lactams in activity against Stenotrophomonas maltophilia (Fass, 1980; Appelbaum et al., 1982).

Many of the cephalosporins that have been most associated with bleeding and coagulopathies, such as moxalactam, cefoperazone, and cefamandole, are rarely used. All contain an NMTT side chain. The cephalosporins in common usage that contain an NMTT side chain include cefotetan, ceftriaxone, and cefepime. However, confusing the issue is the observation that other cephalosporins that do not have an NMTT side chain have also been associated with bleeding disorders. Possible mechanisms of coagulopathies are many: interference with intestinal synthesis of vitamin K2, inhibition of hepatic carboxylation of vitamin K, irreversible inhibition of platelet function (moxalactam), thrombocytopenia, inhibition of fibrin polymerization, potentiation of warfarin effects, and eradication of vitamin K-producing bacterial flora in the gut (24).

The antipseudomonal third-generation agents include ceftazidime and cefoperazone. The increased activity against P. aeruginosa is countered by their diminished susceptibility to ESBL- and AmpC-producing Enterobacteriaceae and gram- positive organisms (MSSA and streptococci). The common theme among them appears to be their association with so-called collateral damage to host and institutional flora. Associations between their use and the selection ofVRE, ESBL- and AmpC-producing Enterobacteriaceae, and Clostridium difficile disease have been reported (89,90). In addition to the vast clinical data, Donskey’s model demonstrates the ability of third-generation cephalosporins to select for and sustain VRE and ESBL-producing organisms (55,91).

Pharmaceutical substance powders of MEM, AVI sodium salt, imipenem, ceftazidime, and 10 antibiotics (except for cefoperazone sodium-sulbactam sodium and piperacillin sodium-tazobactam sodium) in Table 1 were purchased from Dalian Meilun Biotech Co. Ltd. (Dalian, China). Pharmaceutical substance powder of vaborbactam and relebactam were purchased from Shanghai Macklin Biotech Co., Ltd. (Shanghai, China). Cefoperazone sodium-sulbactam sodium for injection were purchased from Pifzer Pharmaceutical Co., Ltd. (USA) and piperacillin sodium-tazobactam sodium for injection were purchased from Hanhui Pharmaceutical Co., Ltd. (Shanghai, China). The stock solutions were prepared as the product specifications required: each powder was dissolved using sterile purified water or absolute ethyl alcohol to achieve a stock solution of 10,240 μg/mL. These stock solutions were divided into small portions and stored at −80°C.

Antibiotics have significant role in promoting digestion in animals, prohibiting bacterial infection, and replacing nutrients (Manzetti and Ghisic 2014, Sisecioglu et al.2011). Cephalosporins are in the class of β-lactam antibiotics and are the most common employed antibiotics in medicine (Kummerer 2009, Harris et al.2012). The clinical consumption of cephalosporins is tremendous, because they have strong bactericidal capability and wide antibacterial spectrum. Due to the expanding worldwide market demand, the production output of cephalosporins rises yearly. Cefoperazone that is used in this study, belongs to the third-generation cephalosporin group that works via preventing the biosynthesis of bacterial cell Wall (Pfaller et al.2017). Cefuroxime is a second generation semisynthetic cephalosporin (Pfaller et al.2017). It has impressive role against Gram-positive and Gram-negative bacteria, and is less susceptible to most β-lactamases. Cefazolin is the first generation semisynthetic cephalosporin and it posseses significance for treatment of different infections including, bones, lungs, stomach, skin, etc. Cefazolin posseses a potential role against Gram-negative bacteria (Harris et al.2012).

A retrospective study on microbial spectrum showed that the majority of bacteremia were caused by Gram-positive bacteria in pediatric cancer patients between 2004 and 2011 in the Netherlands and Switzerland.27 Susceptibility rates were comparable between their populations and ours, and low vancomycin-resistant rates were also observed. Penicillin, amoxicillin, amoxicillin/clavulanate, ampicillin/sulbactam, and erythromycin resistance was high in our patients possibly due to these drugs being widely used in outpatients and general pediatric wards when bacterial infection was suspected. Due to laboratory limitations there was no drug resistance information available for cefoperazone, the first-line antibiotic in our unit. Therefore, the dosage of antibiotic regimens was adjusted referring to the clinical manifestations and other drug sensitivity. In general, cefatriaxone resistance was high while cefepime and ceftazidime resistance was relatively low, which was in line with other reports.28,29
