The fruit and leaves of the tree contain a complicated mix of phytochemicals, including flavonoids, iridoid glycosides, alkaloids, essential oils, fatty oils, diterpenoids, and steroids.4 Among those phytochemicals – some that are shared with other medicinal species and others that are unique to Vitex agnus-castus – are: vitexin, casticin, agnuside, aucubin, apigenin, thujene, pinene, sabinene, myrcene, terpinene, limonene, cineole, linalool, cryptone, citronellol, cumin aldehyde, carvacrol, b-caryophyllene, farnesene, myristicin, and scores of others.5

Julius Angeline from India tested different polyhydroxylated derivatives against ALR2, identifying a small number of compounds showing inhibitory efficacy in the nanomolar range [85]. These include the cyclopenta[c]pyran-5-oles agnuside 43 (IC50 22.4 nM) and picroside II 44 (IC50 130 nM), and the croman-3-ones derivatives eupalitin-3-O-galactoside 45 (IC50 27.3 nM) and 7-O-methylwogonin 46 (IC50 108 nM) (Figure 7). By combining agnuside and eupatilin-3-O-galactoside the author succeeded in obtaining an active phytocomplex which proved to inhibit ALR2 from both ARPE19 human retinal pigment epithelial cell line and diabetic lens, showing IC50 values lower than the commercially available ALR2 inhibitor epalrestat.

In this interventional and prospective study, subjects received one tablet per day (taken before going to bed) containing a combination of soy isoflavones 60 mg (genistein 30 mg and daidzein 30 mg); Lactobacillus sporogenes one billion spores; Magnolia officinalis bark extract (50 mg) equivalent to honokiol 0.75 mg, Vitex agnus-castus L. fruit extract (40 mg) equivalent to agnuside 0.2 mg, vitamin D3 (5 μg), calcium (141 mg) (Estromineral Serena Plus®, Viatris Pharma; hereafter referred to as combined isoflavone compound [CIC]).
