Benzydamine

An 81-year-old woman presented with pruritic, erythematous, scaly and enlarging plaques over the dorsum of her hands and forearms, on her chin and the centre of the forehead. Histopathology showed a dense lymphoid infiltrate in the superficial papillary dermis with focal interstitial and perivascular extension. The infiltrate was largely composed of lymphocytes. Immunophenotyping revealed a T-lymphocyte CD4/CD8 ratio of 5:6. The direct immunofluorescence study was negative. A T-cell receptor gamma gene rearrangement showed a polyclonal pattern. Patch and photopatch testing revealed a positive photopatch to benzydamine 1% pet. The patient now told that for several years she had used a gynecological solution containing 0.1% benzydamine HCl for vaginal hygiene. After ceasing its use, complete remission was achieved without recurrence in the following year. The patient was diagnosed with lymphomatoid photocontact dermatitis from benzydamine (12).

In the earlier days of the application of immunohistological methods to tumor diagnosis, the method used to demonstrate the site of antigen localization was immunofluorescence using the action of ultraviolet light on antibodies conjugated with fluorescein or other fluorogen. Indeed, immunofluorescence is still the method of choice in some areas of study because of its high degree of sensitivity and ability to locate small quantities of antigen. It does suffer from disadvantages in the short storage life of the sections and the inconvenience of requiring a separate facility for ultraviolet light microscopy in addition to the white light microscopy available in most laboratories.

Direct immunfluorescent tests of perilesional skin and/or mucous membranes is the most important investigation. Biopsy of the conjunctiva can be performed by an ophthalmologist; however, the test is not always positive, for the reasons discussed above. Indirect immunofluorescence is positive in only approximately 50% of patients.

All renal biopsy specimens were processed routinely for immunofluorescence microscopy and light microscopy, Sections were stained for direct immunofluorescence with fluorescein isothiocyanate–conjugated antibodies specific for human IgA, IgG, IgM, C3, C4, and C1q. Immunofluorescence results were graded on a scale of 0 to 3, and a score ≥1 was regarded as immunofluorescence positive in this study. Sections used for light microscopy were stained with hematoxylin and eosin, periodic acid–Schiff, and Masson’s trichrome stain. Sections for light microscopy were graded according to the Oxford MEST-C score of IgAN [10]. Two pathologists (T.X. and J.Z.) who were blinded to clinical data reviewed kidney biopsies from all patients independently. All disagreements were finally resolved by the senior pathologist (H.S.).

Immunofluorescence is a technique for detecting antigens and antibodies in which the antibodies are labeled with fluorescent dyes, and the resulting antigen-antibody complex is visualized using a fluorescent microscope. The indirect immunofluorescence assay was used in this research (Odell and Cook 2013). Occludin Antibody (E-5) was obtained from Santa Cruz Biotechnology, which is a mouse monoclonal IgG2b (kappa light chain) (Oregon, USA). Using a Leica fluorescence microscope (Model: Leica DM 5500B, Leica Microsystems, Wetzlar, Germany), tissue sections were examined and imaged in blue, green, and red channels. The fluorometric analysis of the results was performed using Image-J software on a minimum of five fields from each rat testicular segment (minimum of two rats from each group) (NIH, USA).

All of the included studies in this systematic review utilized PCR for hMPV detection. Nucleic acid amplifications are the preferred diagnostic methods for detecting hMPV [39]. The viral culture has demonstrated poor applicability as a result of in vitro slow growth, delayed cytopathic effects and the requirement for exogenous trypsin in the culture medium. Although immunofluorescence is a simple technique, it can lead to false positive results and require experienced personnel [40]. Respiratory virus detection in clinical samples is highly dependent on the sample collection techniques and specimen sources [41]. Most of the studies in the current review used NPS followed by TS, NS and NPA. Although nasal wash (NW) and NPA may be considered superior to swab specimens for respiratory virus detection, obtaining NW and NPA is difficult and unpleasant to the patients [20]. Compared to NW, swab samples are less invasive, easier to collect and can be implemented in physicians’ clinics or emergency rooms. In a large comparative analysis of paired NPS and oropharyngeal swabs(ORS) for diagnosis of eight respiratory viruses there was no difference for hMPV detection [42]. On the contrary, a study by Munywoki et al. found that NPS yielded a higher detection rate than NW for viruses in children [43]. A larger sample size may be needed to determine the significance of these differences.
