Antibodies to anionic (negatively charged) phospholipids or phospholipid-binding proteins (aPL antibodies) are associated with recurrent, usually mid-trimester, spontaneous abortions, autoimmune thrombocytopenia, arterial and deep venous thromboses, and livedo reticularis (141–145). The definition of recurrent miscarriage has varied in the literature, with implications as to prognosis of the untreated pregnancy. The standard obstetrical definition is three or more consecutive losses. However, the definition used for most rheumatologic patients has been two or more losses (sometimes even one). Incidence of pregnancy loss increases with greater number of preceding miscarriages, which has implications in evaluating efficacy of one or another therapeutic approach.

In traditional or religious subcultures, community prayers are supplemented by gifts to pacify annoyed gods, goddesses or ancestors. Ceremonial rituals draw on associative procedures like untying knots, or eating special herbs and rich milk products, seed pods, garlic cloves. An array of symbolic talismans encourages fecundity, from wearing special threads or beads, to dolls, or a bit of placenta to be kept under a pillow during sleep. Where non-conception is ascribed to a curse, in addition to blessings, incantations remedial activities involve exorcism, and usually some form of sacrifice (money, poultry, goats, etc.) to appease powerful spirits. Similarly, if conception results in recurrent miscarriage, interventions may include special nourishment, wearing a consecrated amulet, massaging the recalcitrant belly with plant extracts, application of medicinal concoctions to aid the vaginal outflow of impurities, and even manual trans-vaginal repositioning of the womb.

A cross-sectional study was performed to assess all of the early pregnancy loss samples which were referred to the pathology lab of Avicenna infertility center in Tehran from March 2013 to March 2018. Permission to conduct the study was given by Avicenna ethics committee numbered IR.ACECR.Avicenna.REC.1397.010. All of samples of early pregnancy loss (below 12 gestational weeks) were included in our study. All subjects provided written consent to share their samples’ information for research goals. Demographic and medical characteristics of parents including age, past medical history, previous history of pregnancies and miscarriages, and also parents’ consanguinity was obtained from the medical records in the clinic. The term ‘recurrent miscarriage’ in this study refers to three or more miscarriages in the first trimester. Each first trimester miscarriage in this study was checked for intrauterine embryonal death through calculating the time difference between LMP and gestation time obtained from ultrasound evaluation. The cases in which the time difference was higher than 14 days, and also the ones that showed symptoms of pregnancy product retention in utero, were excluded from the study.

Epidemiological studies have shown that the incidence of recurrent miscarriage is on the rise. With the increase in the number of miscarriages, the risk of re-abortion has also been risen. After two consecutive miscarriages, the third time there is a risk of up to 80% of pregnancy loss [6]. The occurrence of recurrent abortion, especially URSA, causes great psychological trauma to women of childbearing age. Women who have not given birth to healthy children after marriage or have had an abortion bear even more psychological pressure, which will also have a negative impact on the stability of family and society [7].This disease will cause women to have adverse psychological factors such as inferiority complex and anxiety, act on the nervous system of the body, affect the two-way regulation function of the maternal neuroendocrine system, lead to excessive release of neurotransmitters and endocrine hormones, react on the immune system, cause a vicious circle of disease psychology disease, and increase the risk of abortion in second pregnancy by about 2 times [8].

Uterine fibromatosis (UF) is the most common type of benign gynecologic tumors, affecting 20–40% of women of reproductive age [1]. UF affects women of every ethnic group but in African-American women the incidence is three times higher than Caucasian women [2,3]. UF is symptomatic in about 30% of patients; the presence and severity of symptoms depend on the localization, number, and size of fibroids. The most common symptoms include abnormal uterine bleeding, chronic pelvic pain, infertility or recurrent miscarriage, compression symptoms and urinary symptoms (dysuria and urinary urgency). According to frequency and symptoms, UF is a major source of morbidity for reproductive-age women, with a strong negative impact on the QoL. Women with UF experience significant psychological burden, including worry, fear, anxiety, depression, and helplessness. Fibroids can lead to a negative self-image which can also impact relationships [4]. One in four women reports that fibroids negatively impact work life and hinder them in their careers. Between 28 and 40% of women report loss of work due to fibroids. 68–88% of women report embarrassment due to the visibility of fibroids on their abdomen. The femaleness and sexuality are often underestimated topics, very impacted by this disease: 22–51% of women report that fibroids negatively affect their sex lives. 41–44% refer pain in sexual relations and 60% refer also decreased libido. 61% of patients feel limited in their daily activities and relationships with friends and relatives, mainly due to fibroids related symptoms and fatigue sensation [5,6].
