The goal of OMG therapy is to reduce symptoms of diplopia and ptosis and to prevent progression and generalization. The medical and surgical means to achieve this are nevertheless controversial.23 Because of the high risk of generalization after OMG, it is usually recommended to manage the treatment in cooperation with a neurologist, especially during the first 2 years of ocular signs, when generalization most often occurs. A symptomatic, non-pharmacological approach can be sometimes very helpful: patching of one eye, use of prisms and/or of ptosis crutches may be temporary solutions. It is mandatory at the initial stage to manage intercurrent infections, which may worsen, temporarily, OMG signs. The patient is also advised to avoid medications (statins, quinolones, aminoglycosides, anesthetics, curariform agents, botulinum toxin) which may interfere with the neuro-muscular junction.

Medicines for treating MG such as cholinesterase inhibitors and corticosteroids are available but their use is limited due to side effects, while thymectomy, a surgical solution, is beyond the reach of many patients in developing countries.The ptosis crutches that are available on the market present challenges such as interfering with the closure of the eye, which causes ocular surface drying and frequent irritation of the eyes.The open-access, modular and adjustable ptosis crutch developed in the Medical Device Laboratory at UCT provides opportunities for further open innovation to address the limitations of the device.The facility for adjustment along axial, transverse and frontal planes caters for inter-individual variability in horizontal eye position, globe projection and eyelid elevation without causing irritation to the eyelid skin, drying of the cornea, and obstruction to the visual field.
