Consider transcendental/spiritual theories of addiction too. In his book Addiction and Grace (2008), psychiatrist Gerald May says all human beings long for wholeness, completion, and fulfillment. We hunger to love, to be loved, and to move closer to the source of love, God. This longing gives meaning to life. Yet, “life happens,” and modern experience often creates a sense of aloneness, alienation, and pain. Addiction offers temporary relief. Psychiatrists Carl Jung and more recently Stanislav Grof describe craving as the equivalent of the spiritual thirst for wholeness. People also crave release from pain, a desire for joy, a “rush,” a “high.” Spirituality is an important component of 12-step recovery programs.
Analgesics during Pregnancy

Acute narcotic analgesic therapy is most commonly indicated for postoperative pain relief. Women who require surgery during pregnancy can be safely treated for acute pain relief with a variety of analgesic agents for postoperative pain with relative safety for the fetus. Two commonly used regimens are meperidine (Demerol), 50–100 mg IM every three to four hours, or hydromorphone (Dilaudid), 1–2 mg every three to four hours. Caution should be exercised with large doses during breastfeeding. Dosage regimens for various parenteral preparations are summarized in Table 8.3.

Sublingual nitroglycerin is the most effective drug to relieve symptoms during an acute angina attack. It is a strong smooth-muscle relaxant and also a vasodilator. It works primarily in the peripheral vascular tree, primarily of the capacitance or venous system, and in the coronary blood vessels. Sublingual nitroglycerin can also be taken before exertion to prevent an attack. Effective relief occurs in 1.5–3 minutes in most cases, completely by 5 minutes, and lasting as long as 30 minutes. Doses can be repeated every 4–5 minutes, up to three times, if the effects are incomplete. The patient must always carry nitroglycerin, regardless of its dosage form, for prompt use when needed. If tablets are used, they must be stored in a light-resistant glass container that is tightly sealed in order to retain their potency. The drug deteriorates very quickly, so small amounts should be frequently purchased.

Use of eye drops for DED in pregnant women is challenging because ophthalmologists need to consider the possible adverse effects on both the mothers and the fetuses. Because eye drops do not undergo first-pass metabolism in the liver,13 they may have an adverse effect on fetuses even if they are locally administered. The package inserts state that pregnant women should consult doctors and pharmacists before using these medicines.14–16 However, no studies have investigated the safety of eye drops for DED in pregnant women, and thus ophthalmologists may not be able to make informed decisions. Information on the safety of eye drops for DED during pregnancy would decrease concerns about their prescription to pregnant women, thereby providing symptom relief in many pregnant women with DED.
Healthcare professionals’ and budtenders’ perceptions of perinatal cannabis use

Healthcare professionals and budtenders both noted that perinatal women report using cannabis for medical, and not recreational, reasons. Relief from nausea, anxiety, pain, and stress are noted to be the most common reasons perinatal women are using cannabis. I do think that women have less anxiety [when using cannabis]. And I do think it helps them with nausea in early pregnancy. And that’s primarily the two reasons my women use it. Serious side effects from anti-VEGF injections, e.g., eye infections, were not mentioned by patients. Bloodshot and watering eyes, pain, general discomfort and visual distortions were the most reported side effects. With familiarity with the injection procedure, participants developed strategies for the relief of slight side effects: almost all participants needed some rest after the injection, and many would sleep while waiting for the discomfort and visual disturbances to resolve [6,22,29]. The reasons for terminating the therapy, as reported by patients, were: unsuccessful treatment, increasing age, too much effort to organise someone to accompany them to the treatment centre, too much exertion, transportation costs, bad general health, dread of the injection, the burden on relatives or carers, the costs of the treatment, and lack of confidence in the doctor [34]. If treatment was no longer an option for patients, this often felt like a major loss and was not easy to accept [18].
