Immediate loading of dental implants – planning and provisional restauration: A case report

Two implants Vega®NV Ø3,5 mm × 10 mm (Klockner® Implant System SA, Barcelona, Spain) were placed with 42 NCm torque insertion and adequate primary stability was confirmed by resonance frequency analysis (RFA) with the PenguinRFA® (Integration Diagnostics Sweden AB®, Sweden). The latter recorded an implant stability quotient (ISQ) of 78 for both implants, enough to perform immediate load, as will be further discussed.

The sample consists of 146 rehabilitated implants (KL Klockner® and Cone Essential Klockner®). RFA measurements were done with OsstellTM and its respective Smartpegs for each implant type. The stability was defined in units of ISQ (Implant Stability Quotient) between 1 and 100. The smartpeg was attached to each implant with a torque of 4–6 Ncm (Figure 3) and measurements were performed perpendicularly to the long axis of the implant in buccolingual and mesio-distal direction.

In the in vitro study, the reproducibility of the two techniques was determined for each implant using the device shown in Figure 1. Different values of trabecular bone density #10, #20, #30 PCF (PCF corresponds to the notation used by the phantom manufacturer to rank the phantom quality) and cortical thickness were considered to assess the effect of bone quality on the ultrasonic indicator (UI) and on the Implant Stability Quotient (ISQ) values. The effect of the implant insertion depth and of the final drill diameter (1 or 2 mm) was also investigated.

The most significant clinical attribute, which is displaying the effectiveness of the primary stability and the ossification is Implant Stability Quotient (ISQ) which is an integer between 0-100 (Park et al. 2011; Pammer and Bognár 2015; Scarano et al. 2006). It is important to emphasize, that the ISQ system produces authentically information about the implant’s stability, thus patients healing process. Clinicians can make difficult decisions depending on their implant stability measurements.

The Implant stability Quotient (ISQ) measurement was performed by the using of an electromechanical dedicated device (Osstell, Gothenburg, Sweden). The device is a controlled high- sensitive measurement carried on the implant positioned in the bone.
