Bipolar Disorder

Lithium: Lithium orotate, a low-dose source of lithium, is often prescribed for bipolar disorder. Approximately 3.83 mg of elemental lithium is obtained from 100 mg of organic lithium orotate. Lithium orotate can be used in low doses (e.g., 5 mg) with good results and no side effects.9,10 Clinical trials involving 150 mg/day doses of lithium orotate administered 4–5 times a week showed a reduction of manic and depressive symptoms in bipolar patients.10 Lithium orotate is also used as maintenance therapy for patients with a diagnosis of bipolar disorder.11

Considering this patient was on relatively low doses of mood-stabilizing agents, as previously stated quetiapine 400–800 mg/day and lamotrigine 200–400 mg/day, supplemental lithium orotate 10 mg twice daily was added for its mood-stabilizing effects [38,39], felt by some to be comparable to that of lithium carbonate [40,41] but with a possibly more favorable side effect profile including possibly lower risk of lithium toxicity [42,43]. The latter is a relevant point in endurance athletes who might be at risk for dehydration. By definition, supplements and other complementary and alternative treatments have less evidence to support their use as compared to prescription medications. However, this athlete-patient was not interested in taking prescription lithium or another classic mood stabilizer, could not tolerate a higher dose of quetiapine nor wished to substitute it with an alternate agent, and was very motivated and positive about the idea of a supplement option. While non-regulated supplements are not without risk in high-level athletes given their safety is less well established and given that there is a theoretical risk that they may be tainted with prohibited performance-enhancing ingredients, this athlete purchased her product from a reputable source and felt comfortable with the risk.
