Is Fibromyalgia a Central Pain State?

In the first study, nine patients were given 10 mg morphine, 11 lidocaine [5 mg/kg] and 11 ketamine [0.3 mg/kg] intravenously (2). Ketamine [Ketalar® Park-Davis] is an N-methyl-D-aspartate [NMDA] receptor antagonist. The study was placebo controlled. A responder was defined as a person where pain intensity [visual analog scale] was reduced by 50 percent or more. None responded to morphine. The dose of morphine may have been too small. Four responded to lidocaine, whereas eight were responders in the ketamine test. Pressure pain threshold and tolerance thresholds measured by a dolorimeter increased significantly compared to placebo in the ketamine test.

The fourth investigation exploring the link between sexual abuse and psychopathology in fibromyalgia was performed at the University of Alabama, Birmingham (Alexander et al., 1998). The authors recruited 75 women who had been diagnosed with fibromyalgia after being evaluated by the staff of the outpatient rheumatology clinic. Criteria for inclusion were age between 18 and 65 and the absence of other rheumatic disorders or chronic fatigue syndrome. The assessment of sexual and physical abuse relied on a seven-item interview similar to that used by Boisset-Pioro and colleagues (1995). Psychiatric disorders were identified with the computerized version of the Diagnostic Interview Schedule (Blouin et al., 1988). Self-administered questionnaires measured the severity of pain and fatigue, health-related quality of life, coping strategies, and daily environmental stress. Objective evaluations with a dolorimeter assessed the threshold for perceiving pain at 10 musculoskeletal sites.

It is imperative that the doctor do a “point count” on any patient suspected of having FS and that the examiner press with a force of approximately 4 kg. A dolorimeter (Figure 9.1) is an instrument for measuring the precise amount of pressure applied over an anatomic site. It can be used to quantitate sensitivity of tender points. This device is not needed to make the diagnosis of FS but is used to increase the level of objectivity of the examination.
Algometry for the assessment of central sensitisation to pain in fibromyalgia patients: a systematic review

Algometry has proven to be a suitable instrument to assess central allodynia and hyperalgesia in FM, as well as in other pain conditions [11,12]. Algometry is often indistinctly referred to as dolorimetry or quantitative sensory testing (QST), with both of those terms also being used in the pain literature [13,14]. While the tender point count (as used for diagnosing FM) is limited only to the evaluation of certain spontaneous pain points via the application of pressure [2], algometry evaluates and detects abnormal nociception using various types of pain stimulation in different body locations [15,16], providing a more sensitive, in-depth and exhaustive pain evaluation [17,18]. Besides, pain responses evaluated by algometry have shown strong associations with the clinical pain in FM [19], as well as with improved clinical outcomes after intervention [20].

Mechanical allodynia was assessed using a Von Frey test on day 0, 7, 14, and 21. The rats were adapted to the assessment situation for at least 30 min prior to initiation of stimulation. During the assessment, rats were placed on a mesh steel wire, covered with organic glass, and Von Frey electronic dolorimeter (VonFrey electronic dolorimeter 2390, America IITC) was used to sting one of the rats’ post planta pedis, each filament was presented perpendicularly against the paw, with sufficient force to cause slight bending for 2–3 seconds. The stimulation threshold was recorded with rats reaction like licking foot or foot lifting, repeated three times with an interval of 15 minutes, then the average value was calculated as the mechanical hyperalgesia value of rats.
