The APC mutational site has also been shown to be associated with the numerous extra-colonic manifestations of FAP (Figure 24.1) [1]. Congenital hypertrophy of retinal pigment epithelium, found in 70–80% of patients with FAP, is a benign finding of brown to black, round to oval retinal lesions. Also common are dental abnormalities, including supernumerary teeth, found in up to 80% of patients, osteomas found in 50–90%, and soft tissue lesions, such as epidermoid cyst and fibromas, found in 50% of patients with FAP [1].

The best candidates for SHED are moderately resorbed canine and incisors with the presence of healthy pulp. In children, other sources of easily accessible stem cells are supernumerary teeth, mesiodens, over-retained deciduous teeth associated with congenitally missing permanent teeth and prophylactically removed deciduous molars for orthodontic indications (Thomson et al. 1998).

Dental abnormalities such as supernumerary teeth, absence of teeth, un-erupted teeth, odontomas, and dentigerous cysts have been found in about 17% of classic FAP patients, which is high compared to the 1%–2% observed in the general population [4].

Supernumerary teeth are usually associated with genetic syndromes. Several genes have been identified, including RUNX2, TRPS1, NHS, APC, and EVC [43]. However, the etiology of ST is not clear. The most common syndrome associated with supernumerary tooth is CCD [52], approximately 94% of CCD patients have dental anomalies, including ST. Dental abnormalities in CCD include delayed or failure of eruption of teeth, especially the permanent teeth, retention of primary teeth, crown and root abnormalities, high arched palate, underdevelopment of maxilla, delayed or no mandibular symphysis ossification, multiple impacted permanent teeth and also multiple supernumerary teeth [4,57]. Dentin formation (primary and permanent teeth) is normal, although there are problems in shedding of deciduous teeth and eruption of permanent teeth [44]. The reasons for delayed or failure eruption of permanent teeth in CCD are absence of cellular cementum at the root apex, decreased alkaline phosphatase levels and mechanical obstruction [57,58]. The presence of multiple ST in CCD may cause mechanical obstruction and may be a major factor for the impaction of permanent teeth [59].

Among all the dental institutions, the highest incidence of impacted teeth was observed in the maxillary canine region (435 [24.0%] and 411 [22.7%] teeth in the right and left quadrants, respectively) followed by the central incisor region (113 [6.2%] and 112 [6.2%] teeth in the left and right quadrants, respectively), and second premolar region (70 [3.9%] and 65 [3.6%] teeth in the left and right quadrants, respectively). In the mandible, it was particularly high in the second molar region (70 [3.9%] and 55 [3.0%] teeth in the right and left quadrants, respectively) followed by the second premolar region (47 [2.6%] and 42 [2.3%] teeth in the left and right quadrants, respectively) (Table 1). Impacted supernumerary teeth were the commonest (52 teeth) in the maxillary midline region.

Modified Total Dental Index (mTDI; Table 1) [25], and the number of teeth were used to determine the overall oral health of the patients. These parameters were assessed from the panoramic tomography. Panoramic tomography was available for 222 patients, of which 207 were adults. The number of teeth was determined only for adults. Unerupted and supernumerary teeth were also included in the number of teeth.
