In severe cases of hypothyroidism, there is excessive accumulation of mucopolysaccharides in the dermis leading to non-pitting induration of the skin, especially involving the face (Figure 5.6), hands, and pretibial areas. Periorbital puffiness, macroglossia, a loss of the lateral one-third of the eyebrows, and generalized xerosis are associated and help in clinical differentiation.21

During prolonged space flight, the cardiovascular system adapts to the environment of microgravity because orthostasis is not a problem in weightlessness. High-pressure receptors decrease their responsiveness over time, which is why astronauts like to undertake regular exercise in space to prevent muscle and autonomic deconditioning. As the astronaut experiences zero gravity, the 1 G effect forcing blood to the periphery is lost, resulting in a headward movement of fluid that increases cardiac filling and CVP, resulting in facial periorbital puffiness. As a result of the high CVP, natriuretic peptides are released into the circulation, which facilitates the loss of Na+ and water in the urine.

1. Before surgery, eat fresh fish and ‘nokhodab’ (pea porridge, a pudding containing yellow Pisum sativum L., boiled in muslin until it resembles ‘Hummus’ (Cicer arietinum L.)). 2. Surgery ought to be done before cataract becomes mature (obscurity of appropriate density) in a room with dimmed light and closed windows. 3. The operated eye should be covered with a bit of cotton, on which a blend of egg yolk and Viola odorata L. is applied. 4. After ‘ghadh’ (surgery), the patient should lie on his back in a dark place for 3 days, until there is no more feeling of discomfort. The bandage ought to be changed every 3 days. 5. Bathing, drinking, sexual intercourse, and any condition which may lead towards puffy eyes (periorbital puffiness) such as acute rage, headache and coughing should be prevented [34].

For each patient, we collected information on positivity/absence of 16 symptoms of hypothyroidism at the moment of the inclusion in the study: weakness, increase in weight, constipation, sensitivity to coldness, cold skin, dry and rough skin, pale skin, loss of skin hair, fragile hair, edema of the eyes (periorbital puffiness), face edema, peripheral edema, harsh voice, slow speech, dyspnea, memory disturbance. We were interested to identify possible predictors (thyroid autoimmunity and/or hypothyroidism) of each symptom manifestation. Therefore, for each symptom, we used logistic regression model, where symptom status (with/without symptom) was dependent binary variable and TgAb, TPOAb, TSH and T3 levels were independent variables.
