The depressor septi nasi muscle is generally hyperactive and needs to be addressed in surgery. Also, supratip deformity is common in the secondary patients of this group, which has issues with redraping. Reduction should be limited as the skin envelope has limited contraction capacity, and over-resection should be avoided altogether (Figures 7.46–7.48).

Depressor septi nasi muscle: Constricts the nasal aperture by drawing the nasal septum downward and narrowing the nostril openings. Depressor fibers originate from the incisive fossa of the maxilla and insert onto the nasal septum and dorsal portion of the alar part of the nasalis muscle. This muscle sits between the mucous membrane of the lip and the orbicularis oris, as shown in Figure 14-82. Not all individuals have a depressor septi nasi muscle, and it is not clear if this individual had one or not. That said, if one is present, it is more directly accessible by means of an intraoral approach, starting at GV 28.

Botox was injected at the depressor septi nasi muscle, constrictor nasai muscle at the base of lateral crura to make tip definition and up word tip rotation. Botox was injected sometimes to the dilator naris at mid lateral alae, in case, there is flaring in the alae and of course to the bunny lines when it is existing (Figure 1(c)). Dose of Botox was 2–4 units/each injection site.
