The paramedics diagnosed opioid toxicity and administered naloxone. The patient regained consciousness and was brought to hospital where he recovered well. His blood results were sent for toxicological screening and were positive for both fentanyl and heroin. The patient expressed an ongoing desire to abstain from intravenous drug use and was eventually admitted to an inpatient drug rehabilitation unit. He commenced treatment with buprenorphine to support his gradual withdrawal from opioid use (Figure 14.1).

Clinical toxicology laboratories are involved in the detection of drugs in suspected cases of drug abuse or overdosage or for monitoring of drugs in sports. Drug abuse tests are generally noninvasive and employ mostly screening tests for the more common illicit compounds. Urine is used for large-scale testing because of its ease of acquisition and its noninvasive nature. Analysis for unauthorized use of drugs is performed in a variety of settings. Testing of drug abusers in drug rehabilitation clinics and hospitals is done to monitor the effectiveness of the rehabilitation program for the patient. Monitoring of drug use in the workplace, in schools, and in individuals involved in critical jobs (such as police and pilots) is a relatively recent phenomenon. Testing is required or is mandatory to protect employees, employers, students, and the public from the harmful consequences of illicit drug use by individuals responsible for public safety. The identification of drugs in drug abuse cases and monitoring of suspected drug use in sports is generally accomplished by rapid screening of urine samples.
Relationship between anxiety and drug abstention motivation in men with substance use disorders: a cross-sectional study of compulsory isolation rehabilitation in China

Previous research investigating drug abstention motivation focused on its relationship with abuse (Lu et al., 2017), self-concept and self-efficacy (Chung & Maisto, 2016; Majer et al., 2015), and social norms and social support (Dohnke et al., 2011); however, these studies paid limited attention to the relationship between emotion and drug abstention motivation. According to the motivational dimensional model of affect, emotion involves motivation, and emotions with different motivational intensities have different effects on attention and cognition (Gable & Harmon-Jones, 2010). Second, previous studies concerning drug abstention motivation mainly focused on people with substance use disorders in Western society (Gideon, 2010; Laudet & Stanick, 2010; Lu et al., 2017). Theoretically, different approaches to drug rehabilitation have different effects on emotion and drug abstention motivation in drug use disorders, but researches involving people with substance use disorders in the context of compulsory isolation for drug rehabilitation remain insufficient.
Lifestyle redesign program with motivational enhancement for young people with drug use in Hong Kong: a retrospective study

Our findings were consistent with the study conducted by Jafari et al. (2012) that lifestyle intervention is effective in resiliency enhancement and relapse prevention for young drug users. It also provides more insight into the importance of treatment retention on drug rehabilitation. The Spearman’s rank-order correlation analysis showed that program outcomes had a stronger relationship with program characteristics. In particular, participants who completed the LRP and attended more sessions achieved better outcomes in post-intervention abstinence and daytime engagement. There have been similar findings that treatment completion and length of stay show a positive effect on post-intervention outcomes (Zarkin et al., 2002), and better treatment retention leads to better outcomes in drug rehabilitation (Dalsbø et al., 2010). Therefore, this study further highlights the significance of program characteristics in promoting treatment retention that contributes to positive changes in drug users.
Treatment Barriers Associated with Amphetamine-Type Stimulant (ATS) Use in Malaysia

The study has several limitations. Respondents came from a convenience sample of people who were detained in a community treatment programme. The treatment barriers faced by this group may differ from those who have no history of drug rehabilitation or have been legally mandated to attend treatment. A large majority (86%) of the sample had previously undergone drug rehabilitation programmes. so the findings may not be capturing the views of people who use ATS but have not attended a prior treatment programme. Even so, our main findings are consistent with findings of studies elsewhere. It is hoped that prospective studies will attempt to investigate treatment barriers among a cohort of ATS users who have not sought treatment. Additionally, a larger sample of women might shed further light on the barriers that operate specifically among women. Finally, relying on self-reports includes risk of bias, exaggeration, and socially acceptable responses.
