Patient management is a very crucial part of the healthcare sector. It includes several elements like patient clinic relation, examination, diagnosis, and treatment. To have a desirable treatment output, patient management becomes a very important factor. A well-organized system is required to handle a patient from day one. A patient’s medical and social history plays a recognizable role in timely diagnosis and treatment. The traditional way of patient management works by maintaining the health records of patients in physical format. It requires a separate record section every clinical to have a good output. But there are a lot of disadvantages to the traditional method. It’s impossible to store records for long years in an organized way. With an increasing population and increasing patients, patient management becomes a cumbersome task for clinics, so there is a requirement for a technology-based patient management system.

The history of the complaint section from the clinical notes contains the details of surgeon’s interview with the patient. This part describes information on patient’s current and past symptoms, the nature of the pain that the patient is experiencing, timeline of the symptoms, previous diagnoses or medical opinions on the patient’s condition, if there are any, previous related nonsurgical and surgical treatments that the patient received prior to the appointment, and outcome of these treatments, whether they reduced patient’s symptoms or not. Later in the clinical notes, patient’s medical history is listed, including patient’s chronic diseases along with previous surgeries that the patient may have had. Patient history on alcohol, tobacco, and illicit drug use are included in social history section.
