Ureteric duplication

Loss of lower-pole function and/or conversion to total nephrectomy is the most significant complication related to heminephrectomy, with reported rates as high as 10%. This can be due to hilar injury or postoperative torsion of the remaining kidney and is more common in infants. For fully mobilized kidneys, consider nephropexy to prevent torsion of the renal remnant. Additional complications include postoperative bleeding (<5%), urinoma (5−20%, more common in laparoscopic procedures, rarely clinically significant) and damage to adjacent vascular and visceral structures (rare).

A laparoscopic nephropexy was then carried out using a transperitoneal approach, whereby the right kidney was attached to the quadratus lumborum just below the liver using a 3-point fixation to avoid torsion. A 6-month post-operative MAG3 renogram carried out in the erect position showed that the right kidney continued to handle the isotope normally, with a relative split function of 51% (Figure 1C). Crucially, her symptoms had completely resolved.
