Clinical andrology as a separate branch of medicine has largely developed in recent years. Many techniques in modern andrology simply cannot develop further without the contribution of microsurgery, which offers the hope of conception and/or restoration of function to patients previously considered inoperable and, therefore, permanently sterile.1 When there is no sperm in the ejaculate, the FSH is normal,2 only one diagnostic test should be performed — testicle biopsy. If this shows “normal spermatogenesis”, the diagnosis is occlusion or agenesis of the vas deferens, and microsurgery should be planned.

The lack of trained men’s health professionals is a significant challenge for andrology. In 2022, we conducted a national-wide survey of 370 clinicians who attended the annual Vietnam Urology and Nephrology Association congress (data not yet published). The result indicated that 50% of respondents who were seeing male patients with sexual dysfunction reported not having received any training in andrology. In addition, 32.4% of clinicians were having difficulties taking a history of male sexual problems owing to a lack of knowledge or experiences on men’s health. This results in the underdiagnosis and undertreatment of testosterone deficiency, which can have negative impacts on men’s overall health and quality of life. Similar results have been reported in Europe as well, with only a minority of physicians actively investigating their patients’ health [205].

This comparative cross-sectional study was conducted at the consultant medical centre for dermatology, andrology and infertility in Ibb city, Yemen, from March 2015 to August 2015. Samples were collected from men who visited the Andrology clinic for fertility evaluation. The sample size was calculated using OpenEpi software version 2.3. To achieve 80% power and a level of significance of 5%. The sample size was similar to a study conducted by Hanafy et al. (2007). Hence, our study required minimum sample size of 24 patients and 24 control. However, after clinical investigation and interviews, the study involved 30 infertile men of reproductive age (18–50 years old) with abnormal semen parameters and unknown unexplained causes for infertility. The control group involved 30 age-matched fertile volunteers from normozoospermic healthy men. The exclusion criteria were as follows: (a) males aged >18 or <50 years, (b) a history of congenital or acquired disorders of the reproductive organs, (c) clinical varicocele or genital tract infections, (d) patients suspected to have obstructive azoospermia, (e) cases with features suggestive of hypogonadism including repeated low total testosterone and clinical signs and symptoms, (f) history of taking drugs known to compromise male fertility or affecting their sex hormones, (g) exposure to environmental toxicants, excessive heat or irradiation, (h) history of heavy smoking, alcohol intake or drug abuse, (i) cases showing agglutination of their sperms during semen analysis, (j) chronic systemic diseases, and (k) severe obesity (BMI ≥ 40 kg/m2).

From June 2019 to December 2020, 227 patients were evaluated who complained of PE sought medical counselling at the ambulatory clinic of the Urology and the Andrology departments at Assiut University Hospital. For study inclusion patients had to fulfil the lifelong PE criteria according to the ISSM 2014, with ejaculation taking place at ≤1 min of vaginal penetration, failure to defer ejaculation and lack of sexual intimacy leading to adverse personal outcomes. In addition, the patients had to be aged between 20 and 35 years, with average body mass index, married with stable sexual relationship with an intercourse frequency of two to three times per week for at least a month before the study. The patient also had to have complained of PE since his initiation of sexual activity, which was ≥6-months duration, with good erections (five-item version of the International Index of Erectile Function [IIEF-5] score of ≥22), not hypertensive or diabetic or have any comorbidities, and not receiving any medication.
