The Enhanced Recovery After Surgery (ERAS) Society has provided their fourth updated evidence-based care pathway for the management of patients undergoing colorectal cancer surgery. These guidelines contain elements commencing pre-admission through to the postoperative period, and are designed to reduce the stress of the patient in the perioperative period while maintaining physiological function and accelerating the recovery of the patient [8].

Perioperative management of neuroendocrine tumors is quite challenging in elderly patients. Endonasal transsphenoidal surgery, being minimally invasive, is usually well tolerated. Perioperative complications are more severe and are influenced by age-related decline in physiological reserve. A multidisciplinary team approach including surgeon, anesthesiologist, and endocrinologist is must in perioperative period for preoperative optimization and postoperative care.

It is estimated that the prevalence of hypertrophic cardiomyopathy in the adult population is 0.2%. Patients with this condition typically have marked degrees of LV hypertrophy (LVH), reduced ventricular compliance, hyperdynamic LV systolic function, and systolic anterior motion of the mitral valve leaflet, with or without a dynamic pressure gradient in the subaortic area. It is important to identify this condition preoperatively since there may be increased risk of hemodynamic compromise in the perioperative period. AF is also more common in patients with hypertrophic cardiomyopathy. The overall prevalence of this arrhythmia among patients with hypertrophic cardiomyopathy is greater than 20% and the occurrence of AF increases with age.
Comparison of perioperative outcomes and anesthetic-related complications of morbidly obese and super-obese parturients delivering by cesarean section

We included parturients with a BMI ≥40 kg/m2 undergoing cesarean delivery between 1 January 2014 and 31 December 2019. The exclusion criteria were a gestational age <24 weeks, fetal death in utero, placenta accreta spectrum, polyhydramnios, and multiple gestations. The hospital numbers of patients were retrieved from the anesthetic department database. Detailed obstetrical and anesthetic data of eligible patients were then collected from our hospital’s medical record center, which stored records in electronic files. Patients were withdrawn from the study if data were incomplete or missing. We recruited 494 consecutive patients with morbid obesity or super obesity who underwent cesarean delivery. Patients were stratified into two groups: a morbid obesity group (BMI 40–49.9 kg/m2) and a super obesity group (BMI ≥50 kg/m2). The primary outcome was anesthetic complications occurring during the perioperative period (from the operation day to hospital discharge).

In summary, wrist-ankle acupuncture can prevent the occurrence of hypertension in patients after intubation during induction of anesthesia, which is safe, effective, minimally invasive, easy to operate, and easy to promote in clinical practice. Moreover, compared with traditional acupuncture therapy, no discomfort such as soreness, swelling and pain will be experienced during wrist-ankle acupuncture therapy, and the needles are suitable to be left in the patient’s body without interfering with the surgical operation. It produces a continuous intervention effect in the perioperative period, which is conducive to improving patient compliance for popularization, and has important clinical significance in reducing and preventing adverse effects brought about by surgery and anesthesia, and can also improve the prognosis of patients and postoperative quality of life.

Table 2 summarizes the main clinical indexes of the patients during the perioperative period. There was no significant difference in the main clinical indexes before and during operation between the experimental group and the control group, including preoperative albumin, preoperative hemoglobin, preoperative CRP, preoperative CA19-9, preoperative CEA, preoperative lymphocyte count, TNM staging, intraoperative lymph node dissection range, intraoperative blood loss in the range of gastrectomy, operation time, ASA. Among the postoperative clinical indexes, there was no significant difference in CRP on POD3 between the experimental group and the control group. In this study, there was no significant difference in postoperative hospital stay between the experimental group and the control group.
