Those identified as miscellaneous include comments about spontaneous orgasms as well as orgasms from painting, cutting hair, or doing absolutely nothing, among other stimuli (Table 3). Two commenters described orgasms occurring while performing oral sex which, although in a sexual context, seemed to be triggered by hearing a partner orgasm in one case and, in the second, a sense of controlling a partner’s sexual response and eventually just the fantasy of performing oral sex on a partner. One person described experiencing orgasm from intellectual stimulation (Table 3).

SSRIs were rarely reported to be associated with priapism, spontaneous orgasm or increased sexual desire. Such effects were also seen with tricyclic antidepressants, e.g. clomipramine [28,83]. Some patients could have treatment-induced SD in association with agents of low SD-risk profile, e.g. bupropion [84].
