This plant is belongs to the Rutaceae family. Antiobesity effect of C. aurantium contains synephrine, a stimulant with comparable characteristics as caffeine and ephedrine. It is observed to have similar effects by increasing metabolism and energy expenditure and suppressing appetite (Haaz et al., 2006). C. aurantium aided in weight loss and increase thermogenesis to some extent. On the contrary, the loss of fat mass in the test group was appreciably superior compared to the placebo and control groups (Suryawanshi et al., 2011).

Certain dietary factors appear to be associated with the onset of the attack in migraine patients; chocolate, cheese, or citrus fruits are most frequently quoted foodstuffs. Oranges are known to contain considerable amounts of octopamine and N-methyloctopamine or synephrine (Figure 9). Apart from tyramine, there may be other compounds related to migraine. Patients suffering from dietary migraine excrete significantly less free tyramine and far less conjugated tyramine than control subjects. It has been suggested that dietary factors may precipitate an inborn defect in the metabolism of tyramine and related factors.

Gal-3 has been targeted by both natural and synthetic compounds. Synephrine is an active compound made from citrus tree leaves and is said to have anti-cancer properties. One study showed that targeting esophageal squamous carcinoma cells with synephrine can downregulate Gal-3, which inactivates the AKT and ERK pathways [57], but its precise action on Gal-3 is unknown. Additionally, RN1, a polysaccharide purified from the Panax flower, can also bind to Gal-3 and suppress the Gal-3-associated signaling pathway activity shown in pancreatic ductal adenocarcinoma cells [58]. Furthermore, modified heparin derivatives can abolish the Gal-3-mediated augmentation in lung metastasis of human melanoma and colon cancer cells by inhibiting Gal-3 ligand binding [59].

In this case study as in other case reports, the authors failed to review the literature, citing another case study and no current literature. The authors stated that “the patient’s condition could be attributed to his use of synephrine-containing weight loss and performance–enhancing supplements.” The authors further noted that mixing caffeine with synephrine can “lead to an increase in the adverse effects profile of each,” contrary to recent research (Unnikrishnan et al. 2018), while citing an article involving a product that contained ephedrine. No link or direct association with p-synephrine was demonstrated.
