In general, these types of orthoses are made for the individual by an orthotist from polypropylene materials. They are often used to provide support to the spine and prevent secondary bony deformity in situations where there is muscle weakness. In individuals where spasticity is the overriding problem, spinal braces may not be suitable due to the significant muscular forces that can be present, which in turn may lead to problems with pressure areas. Spinal braces can also be difficult to fit, uncomfortable and impact on breathing. Use of customised seating with individualised truncal and pelvic support can be a more beneficial and comfortable option for these situations.

An orthotist is someone who measures, designs and fits orthoses (external devices that can be applied to correct a deformity, rather like splints). This patient has a right-sided foot drop and so will need an orthosis to prevent the ankle from dropping and impeding walking. Physiotherapy will also be required.

Avoiding unnecessary in-person patient evaluations became essential (Hollander and carr, 2020; Smith et al., 2020; Uscher-Pines et al., 2016). As with all National Health Service (NHS) out-patient services, orthotic services were tasked with adapting to the restrictions imposed by the pandemic and asked to adopt new ways of delivering clinical care. Orthotists are registered with the Health and Care Professions Council (HCPC) and are responsible for providing external engineering solutions for patients with neurological, muscular and skeletal pathologies, working in a variety of settings including hospitals and rehabilitation centers.
