Hair extensions have become increasingly popular worldwide for cosmetic as well as medical purposes. They are composed of synthetic or human hair, which can be attached to existing hair with glue, braids, sewing, or clips. The cuticles of the extensions must be aligned in the same direction of the natural hair and they are usually no longer than twice the length of the person's natural hair for a cosmetically harmonious result. Among local side effects, traction alopecia is quite common, secondary to the pulling force applied to the hair follicle over time. It can be induced by sustained tension of the extension on the natural hair, leading to loosening of the hair shaft from its ostium, which may eventually lead hair loss (Figure 14.6) [38]. Hair matting can occur. Pruritus, tenderness and local pain can also be referred by the patient. Allergic contact dermatitis or contact anaphylaxis can arise after sensitization to glues or natural rubber latex that is incorporated into the adhesive for hair extensions [39].

Traction alopecia is extremely common in women and children of African descent, in whom it affects the marginal scalp, particularly the frontal and the temporal scalp. In the early phase, the alopecia is reversible but it becomes permanent in longstanding disease. The bald patch typically mantains some vellus hairs and presents a rim of preserved hairs at the hairline (Figure 12.24).

It is also important to ask about the diet in general (well balanced/vegetarian, vegan or pescetarian), family history of hair loss, and hair care practices. Unbalanced and restrictive diets or strict vegetarian or vegan diets can implicate iron deficiency or other deficiencies (zinc, selenium, vitamin B12, etc.). A positive family history of alopecia areata or androgenetic alopecia may point to a genetic predisposition for hair loss. Some hair care practices (e.g., bleaching, back combing, permanent waving, hot combs, hot blow drying, and straightening) may result in structural defects and hair breakage. Furthermore, heavy extensions, dreadlocks, cornrows, tight ponytails, and braids can lead to traction alopecia.

Male and female AGA are the most commonly studied conditions, but others include female pattern hair loss, traction alopecia, and alopecia areata (24,37,40,44,46,54,57,59,61,63,64,66,80,96–101). While, in clinical trials, oral minoxidil predominantly caused hypertrichosis, postural hypotension, and edema; topical minoxidil was mainly associated with scalp pruritus and hypertrichosis (24,37,40,44,46,54,57,59,61,63,64,66,80,96–101). With topical minoxidil (foam and solution), hair shedding may be observed in the first couple of weeks, paradoxically confirming the drug's effectiveness (66,68). If it continues for more than two weeks, a physician should be consulted (68). This shedding may occur due to the rapid shift in the hair cycle from telogen to anagen phase (68).
Ocular injuries resulting from commercial cosmetic procedures

Various ocular injuries resulting from the treatment have been reported. Traction alopecia of the natural eyelashes can occur with repeated treatments as the hair follicle is damaged with constant traction. This results in weaker and fewer hairs over time. As sharp pieces of the synthetic material fragment, they can deposit under the eyelid and cause corneal abrasions. In a more severe case, synthetic fibres were found deposited within the subconjunctival space, causing an intense inflammatory response and pain.2018 Removal of the prostheses was performed under local anaesthesia in the operating theatre.
Safety concerns when using novel medications to treat alopecia

Sethi and Bansal performed follicular unit extraction in 26 patients with androgenetic alopecia, one patient with traction alopecia, and two patients with scarring alopecia. Two patients experienced persistent pain due to perineural inflammation in the donor region. The pain subsided after two to three months [47].
