Thumb hypoplasia ranges from a slightly small thumb to complete absence (see Figure 15.8f). Thumbs are reconstructable if there is a basal joint, requiring, in the worst cases, MCP joint stabilization (by ulnar collateral ligament reconstruction or chondrodesis), widening of the first web space using local flaps, and an opposition transfer (usually the Huber transfer using abductor digiti minimi) When there is no basal joint or the thumb is absent, pollicization of the index finger can be performed (as long as the index finger is not also hypoplastic, when it will not make a good thumb).

Radial aplasia is characterized by partial or complete absence of the radius and/or radial ray structures [61]. The hand appears clubbed and deviated radially, the radial carpal bones are deficient, and there are varying degrees of thumb hypoplasia, the most common of which is complete absence [62]. The role of MRI is limited in diagnosing limb deformities since fetal motion produces artifacts on MRI. When prenatal US raises suspicion of a presumptive diagnosis of a skeletal dyspalsia, a low-dose CT scan may be performed, adding augmented and high-resolution imaging compared with that of fetal MRI, leading to a more accurate antenatal diagnosis [60, 63] (Figure 17 and 18).

A variety of such malformations involve the thumb, causing possible functional disturbances of varying severity as the thumb is crucial for proper hand function. Thumb hypoplasia/aplasia, with a “non-functioning”, thumb represents a surgical challenge and usually requires a pollicisation procedure, while thumb duplication poses minor surgical and functional problems in the hand.
