Different clinical variants of mastocytosis display different dermoscopic patterns. A uniform light brown pigment network typifies maculopapular mastocytosis (urticarial pigmentosa) (Figure 12b.12). Telangiectasia macularis eruptiva perstans is dermoscopically characterized by reticular vessels on an erythematous/brownish background, sometimes associated with a brownish network.36–39 Dotted vessels and thin and tortuous linear vessels might be seen in both types. Solitary mastocytoma displays diffuse orange to yellowish color or multifocal yellow-to-orange areas on dermoscopy. Overlap of dermoscopic criteria does exist among different types of the disease.36–40

Multiple dermatoscopic features have been described in mastocytosis, depending on the clinical subtype of the disease. As a result, overlapping criteria are frequent among the several subtypes. Specifically, a light-brown blot and a uniform pigment network typify maculopapular mastocytosis, while a reticular vascular pattern on an erythematous/brownish background characterizes telangiectasia macularis eruptiva perstans. Dotted or thin linear vessels may be present in both types. Solitary mastocytoma displays diffuse orange-yellow color or multifocal yellow-orange areas (Figures 6.37 and 6.38). The reticular vascular pattern has been related with more severe symptoms and indicates an increased risk for continuous medication.

In the adult, the papular variety is somewhat like the juvenile form except that it persists. Another adult type is telangiectasia macularis eruptiva perstans of Parkes Weber. Clinically, in early adult life, pink or pink–brown telangiectatic macules start to appear, persist and increase in number over the years.
Delayed diagnosis of adult-onset mastocytosis

Cutaneous mastocytosis is a rare disease, mostly manifested in adults as urticaria pigmentosa, diffuse and erythrodermic mastocytosis, and telangiectasia macularis eruptiva perstans. The affected skin is characterized by Darier’s sign. Extracutaneous involvement should be carefully considered in adults with cutaneous manifestations.1
