Generalized bullous fixed drug eruption

Generalized bullous fixed drug eruption (GBFDE) is a blistering skin disease that can be caused by certain drugs. It is a rare condition that can be induced by iodinated nonpolar radiocontrast media (iohexol) and is characterized by the formation of blisters on the skin. Other drug-induced skin conditions that can occur include erythroderma, exfoliative dermatitis, acute generalized exanthematic pustulosis (AGEP), pemphigus vulgaris, bullous pemphigoid, Stevens-Johnson syndrome (SJS), and toxic epidermal necrolysis (TEN).From: Skin Reactions to Drugs [2020], Contrast-induced generalized bullous fixed drug eruption resembling Stevens-Johnson syndrome [2019]

The oral challenge test is the most reliable method to detect a causative agent. A small dose, one tenth to one fourth of a single dose, is usually enough to provoke an eruption within a few hours. In generalized bullous fixed drug eruption, the initial test dose has to be one tenth or less of a single dose. The drugs that most frequently cause fixed drug eruption are listed in Table 4.

In the differential diagnosis of these severe cutaneous reactions, clinicians should also consider another entity, acute generalized exanthematous pustulosis (AGEP) [97], as well as defined dermatological entities such as erythema multiforme, erythroderma and erythematous drug eruptions, generalized bullous fixed drug eruption (FDE), phototoxic eruptions, staphylococcal scalded skin syndrome, paraneoplastic pemphigus, and linear IgA bullous dermatosis.
