To become a qualified TCM practitioner in the United States, 3–4 years of full-time postgraduate study at an accredited educational institution are required. Most states require national board certification by the Certification Commission for Acupuncture and Oriental Medicine. Because of research supporting the benefits of acupuncture, the introduction of acupuncturists into the healthcare system is becoming more accepted. We strongly advocate incorporating acupuncture into the healthcare delivery system, including coverage by insurance.
Negligence and professional responsibilities

Verbal express consent. After an explanation is given of the intended treatment, a client may verbally consent to the procedure. For example, an acupuncturist might indicate that a number of needles would be sited in particular parts of the body, and describe the physical sensations that would be felt and the impact of the procedure. The patient could then consent verbally to this procedure. This form of consent will commonly be relied upon by practitioners.

AAOM (see Figure 3.4) is the umbrella organization representing the profession of acupuncture in the United States; it assisted in the formation of the National Commission for the Certification of Acupuncturists and the National Council of Acupuncture Schools and Colleges. The Web site has broadened its scope to include such areas as Chinese herbology, qi manipulation techniques, room arrangement, martial arts, and acupuncture. There is an excellent overview of Oriental medicine. Note the sections “How Acupuncture Works” and “How Acupuncture Feels.” This Web site is a reliable resource for referral to certified acupuncturists.Pro-Cultura’s Web Site on Tibetan Medicine<http://www.tibetmedicine.org>

We applied abdominal acupuncture, an acupuncture modality which has been shown to be effective for depressed women in our previous study [15]. The acupoints applied in this study were: Zhongwan (RN12), Xiawan (RN10), Qihai (RN6), Guanyuan (RN4), Shangqu (KL17), Shangfengshi Point (extra-point) and Qipang (extra-point) (Figure 1). An acupuncturist with more than 5 years of experience performed all acupuncture treatments. Before acupuncture administration, each participant was asked to lie in a supine position and to wear a mask over her eyes. Then, the participant’s abdomen was exposed and the skin was disinfected at the acupuncture points. Afterwards, an acupuncture specialist inserted fine needles (0.22 mm × 40 mm) through short plastic tubes or sheaths. The patient’s abdomen was covered with a basket underneath a sheet throughout the treatment period. The needles were left in for 20 min. Patients received abdominal acupuncture once a day for the first 3 days and subsequently once every 3 days. For sham acupuncture, patients took the same position with a mask over their eyes, and the same acupoints were disinfected. Later, short plastic needle sheaths containing no needles were tapped against the skin of the acupoints, but no needles were inserted through the sheaths, and then the patient’s abdomen was covered with a basket underneath a sheet during the ‘treatment’ period to prevent patients from touching their abdomens. The time and frequency of the sham abdominal acupuncture treatments were the same as those in the verum acupuncture group.

Chronically elevated urea levels can cause acute pericarditis. However, it is rare for the acute renal failure to cause pericarditis. Physicians should, therefore, consider for other causes of acute pericarditis in such patients.Patients with signs of sepsis, evaluation for purulent pericarditis should include the history of acupuncture among other common causes (IV drug abuse, history of needle sticks, cardiac surgery). While being a useful tool for pain alleviation, acupuncture does pose some serious threats, especially when attempted at the hands of inexperienced personnel. Hence, with proper education and training of the acupuncturists, to avoid inserting the needle into some high-risk regions and use of appropriate sterile techniques may decrease the incidence of these complications.People opting for acupuncture should be questioned regarding any prior history of valvular heart disease, to minimize the risk of developing infections later on. No guidelines as of now exist recommending antibiotic prophylaxis with heart valve issues seeking acupuncture and may need further studies.

A myriad of professionals devoted to improving symptoms and side effects of cancer lease space in the Cancer Center area. For example, patients seeking nonpharmacologic interventions for symptom management can be treated by acupuncturists who apply principles of Chinese medicine. The acupuncturist is an independent practitioner who is licensed and works with patients by referral from a physician or by self-referral. The cancer program medical director provided guidance in choosing the right holistic practitioners for the cancer center to ensure that their philosophies and therapies were not in opposition to traditional medicine.
