As easily conceivable, data for the newer second-generation Bcr-Abl-I nilotinib and dasatinib are sparse. In a case report by Abruzzese and colleagues, a 33-year-old male patient with CML who was enrolled in the GIMEMA NILIM trial (alternated nilotinib/imatinib), conceived after 40 days of nilotinib treatment and had a healthy boy born at term [36]. The investigator brochure (issued on 8 June 2012) refers to a total of 36 cases of drug exposure via the father. One of these cases presented with fetal abnormalities and ended with a therapeutic abortion [36]. Considering maternal exposure, nilotinib investigator’s brochure reported 45 cases of drug exposure during pregnancy [36]. There was only one case with fetal abnormalities. In addition, there was one pregnant female carrying twins who was exposed; one twin experienced congenital transposition of great vessels and resulted in death, and the other twin experienced a non-serious heart murmur [36].

An 81-year-old female presented to the emergency department (ED) with chest pain. She had a past medical history of type II diabetes mellitus, hypertension, and polycythemia. She did have a family history of coronary artery disease. She was evaluated in ED as non-ST elevation myocardial infarction due to elevated troponins. The patient was initially administered antiplatelets and anticoagulation. On examination, the patient had hypoxia with a saturation of 80% on room air, cyanosis, mild JVD, edema and ejection systolic murmur. On further questioning, patient had dyspnea on exertion, mild swelling and cyanosis of extremities with exercise intolerance. The echocardiogram revealed a single ventricle which was thought to be left ventricle with possible transposition of great vessels. (Figure 1)

Retinoids are a specific family of vitamin A derivatives including several subtypes such as first, second and third generation. Retinol, retinal, tretinoin (retinoic acid), isotretinoin, and alitretinoin are first generation retinoids. Etretinate and its metabolite acitretin are second-generation retinoids (4). Adapalene, bexarotene, and tazarotene are third generation retinoids (5). Retinoids are used in several conditions such as acne, psoriasis and cancer. One of these retinoids is isotretinoin (13-cis-retinoic acid), which was licensed in the United States in 1982 under the brand name Accutane (2). In 1985, Lammer et al. summarized outcomes of 154 pregnant patients with the exposure to isotretinoin (2). After excluding elective and spontaneous abortions, analysis of 21 malformed infants showed that most common craniofacial malformation was microtia or anotia. Maldevelopment of facial bones and calvarium, micrognathia, cleft palate, flat and depressed nasal bridge, hypertelorism, cerebellar hypoplasia, agenesis of the vermis were described cranial anomalies. They also reported several forms of conotruncal malformations such as transposition of great vessels, tetralogy of Fallot, double-outlet right ventricle, truncus arteriosus communis and supracristal ventricular septal defect in conjunction with abnormalities of thymic morphogenesis (ectopia, hypoplasia, aplasia) (2). In our case, fetal anomalies were observed in most of the above-mentioned regions.
