Obturator externus originates from the structures that surround the external/anterior surface of the obturator foramen including the pubic ramus, the ischial ramus, and the obturator membrane (Standring 2016). It inserts via a tendon onto the trochanteric fossa of the femur (Standring 2016).

Attachments: It arises from the anterior surface of the obturator membrane33 and surrounds the anterior bony margins of obturator foramen34 from the inferior pubic ramus and the ramus of the ischium. It also arises from the medial two-thirds of the outer surface of the obturator membrane and from the tendinous arch which completes the canal for the passage of the obturator vessels and nerves.

To expose the paravesical cavity, 0.9% saline was injected into the area between the pubocervical fascia and the bladder. A longitudinal incision in the anterior vaginal wall was made followed by a subfascial lateral dissection towards the pelvic sidewall up to the iliac spine. The needle guides of anterior vaginal wall mesh were used to guide the mesh arms through obturator membrane for the anterior compartment reconstruction. Then, four arms of the tailored mesh were inserted into the pelvic sidewalls and the four arms were pulled out of the skin through the obturator membrane. Then, the four external arms of mesh were cutoff and the skin incisions were re-sutured.
