In the adult, distal humeral fractures occur following a fall on the outstretched hand. The clinical features are as for children, but there may be more comminution and angulation of the fracture fragments.

In conclusion, the novel customized design concepts with minimal screw holes, lateral-medial linking screw and lateral brim were found to provide more initial stability with higher axial stiffness than standard osteosynthesis plates. Addition of lateral brim to single customized plate and addition of lateral-medial linking screws to double customized plate are suggested. In general, double orthogonal plating could provide better fixation rigidity as compared with single plating system. However, customized design in single plating could provide sufficiently good initial stability close to double orthogonal plating, while achieve adequate interfragmentary strain for secondary fracture healing. Use of patient-specific fixation plates together with selective laser melting technology for manufacturing of custom-made distal humerus plates could be considered as promising alternative to the treatment of distal humerus fracture. Further biomechanical studies are needed for a decisive conclusion.
