As revealed by electron microscopy, the RNFL contains bundles of nerve cells (nerve-cell axons) that run just under and across the surface of the retina. A nerve fiber bundle contains four different kinds of approximately cylindrical structures: a) axonal cell membranes around nerve axons (axonal membranes), b) microtubules, c) neurofilaments, and d) mitochondria. Those structures lie in a rather complex cellular environment (some aqueous solution of proteins and other cellular constituents). Within the scope of this chapter, it is interesting to describe briefly those structures and their sizes.24

The RNFL (retinal nerve fiber layer) is the area surrounding the optic disc. For RNFL extraction a modified mathematical method has been used. This method is based on subtracting the whole image from the disc area. Retinal OCT images can detect the structural damage prior to any detectable visual field defects; thus it is effective for early glaucoma diagnosis. Figure 3 shows a retinal OCT image exposing the different retinal layers; ganglion cell layer (GCL), retinal nerve fiber layer (RNFL), and inner limiting member (ILM) have been affected by glaucoma. Analysis of the retinal nerve fiber layer (RNFL), as well as the parameters RNFL thickness and RNFL volume, are significant in discriminating the timely diagnosis of glaucoma.
