In the preclinical and phase I studies, Myocet® was demonstrated to have a circulation half-life of about 3 times that of free DOX, although these results were derived from a wide range of plasma elimination half-lives [72]. Batist et al designed a multicentric clinical trial in which they compared Myocet® at 60 mg/m2 and cyclophosphamide at 600 mg/m2 with an identical dose of DOX and cyclophosphamide every 3 weeks in 297 women with metastatic breast cancer. Treatment was continued until progressive disease or prohibitive cardiac toxicity. There was no significant difference in response rate (43% versus 43%), median progression-free survival (5.2 months versus 5.5 months), or overall survival (21.2 months versus 16.4 months). Patients treated with Myocet® significantly showed reduced risk of cardiac toxicity, less mucositis, and absence of palmar plantar erythrodysesthesia while maintaining anti-tumor efficacy [73].

SCLC is more sensitive to chemotherapy and radiation and often results in excellent imaging responses. A complete response rate of 56% and partial responses in 31% of patients have been reported after standard of care radiation therapy of 45 Gy in 1.5 Gy fractions delivered twice per day (BID) given with concurrent cisplatin and etoposide, resulting in a 5-year overall survival of 26% [26]. Only 10% of patients had stable disease or progressive disease [26]. The primary tumor response of limited disease SCLC to chemo radiotherapy (CRT) also correlates with the duration of brain-metastasis-free survival [27]. For SCLC patients with initial complete response to CRT the median brain-metastasis-free survival averaged 21 months after PCI versus 16 months in patients without PCI [27].

Alzheimer’s disease is a progressive disease that destroys memory and other important mental functions. At first, someone with Alzheimer’s disease may notice mild confusion and difficulty remembering. Eventually, people with the disease may even forget important people in their lives and undergo dramatic personality changes. Figure 25.1(a), represent a cross-section of the brain as seen from the front. The cross-section on the left represents a normal brain, and the one on the right represents a brain with Alzheimer’s disease. Changes caused by the disease has been displayed. For empirical illustration, consider the kernel estimators proposed for residual Mathai-Haubold entropy function. Here we consider the real data sets reported by Bjerkedal (1960) and it represents the survival times of guinea pigs injected with different doses of tubercle bacilli. Guinea pigs are known to have high susceptibility to human tuberculosis. Even an infection initiated with a few virulent tubercle bacilli will lead to progressive disease and death. We consider the data sets obtained under regimen 5.5 and there were 72 observations. AMD is a progressive disease. The early and intermediate stages are usually asymptomatic, characterized by the accumulation of yellow granular deposits beneath the outermost layer of the retina, the retinal pigment epithelium (RPE) (Figure 1(a)). Advanced or late AMD is defined either by the development of atrophy of the RPE and the overlying photoreceptors or by the development of new blood vessels (neovascular membranes) beneath or above the RPE (Figure 1(b)). These new vessels tend to leak or rupture, with subsequent exudation or haemorrhage accumulating in different retinal layers [4]. These two forms of advanced AMD, the former usually referred to as geographic atrophy (GA), and the latter as exudative, wet, or neovascular AMD (nAMD), can occur alone or together, either simultaneously or sequentially, and both forms can lead to significant visual impairment [5]. There is currently no approved treatment available for GA [6]. Intravitreal anti-vascular endothelial growth factor (anti-VEGF) therapy is the mainstay of nAMD treatment [7].
