The Advanced Multimodality Image Guided Operating (AMIGO) suite comprises a leading edge medical and surgical research platform, which integrates an extensive complement of state-of-the-art imaging and interventional surgical instrumentation between three rooms, to facilitate precisely guided interventions. A patient transfer board enables the safe transfer of the patient between the MRI, PET/CT, and operating rooms (Brigham and Women’s Hospital).

At 24 months post-op, improvement of triceps strength enabled the patient to perform a complete lift off the chair. He has initiated transfers with aid of his caregiver and a transfer board, as well as mobilizing his chair completely and independently. The weakness in right-hand finger extensors (MRC 1) is most likely due to surgical considerations, (use of a graft). Longer distances for re-innervation and use of grafts are associated with less optimal outcomes, but are an inevitable necessity in certain situations. On the other hand, strength in left-sided finger extensors, allow the individual to release grasp. Strength of thumb, index and middle finger flexors bilaterally, allow the patient to grasp, hold a pen, send text messages and assist catheter removal. Strength measure with dynamometry and trunk control test, assessed post-surgery showed no changes.

Michael Boninger started this session by describing how repeated wheelchair transfers are an important contributing factor to upper limb pain and pathology frequently experienced by persons with chronic SCI.20 Based upon this centers’ work, skills training to improve transfer techniques could play a critical role in reducing shoulder pain.20, 21 Key recommendations included the importance of reducing and spreading the amount of transfers throughout the day and utilizing appropriate transfer techniques and equipment (e.g., transfer board).
