Pincer nail refers to the over-curvature of the nail resulting in pinching the nail bed. It may affect only the great toe or all toes and cause distress to the patient due to pain. The nail thickens and turns inwards, pinching the nail bed and causing difficulty to cut it. There may be associated bone pathology like exostosis and hyperostosis (Figure 25.15a and b).

The treatment of pincer nails is either conservative with nail softening, braces, and clips,115,116 or surgical with either phenolization of the lateral matrix horns or nail narrowing plus a nail bed plasty.113,117,118

Pincer nail has an incidence rate of around 0.9%, and generally affects the hallux toenail (1). Patients present with a characteristic narrowing of the nail bed and an increase in the maximum transverse curvature at the distal end of the nail (2). While there are several causes of pincer nails (including ill-fitting shoes and osteoarthritis) (1), the resulting transverse overcurvature occurring in each case may cause symptoms ranging from mild discomfort to intolerable pain (3). This, in turn, affects the ability of patients to walk or wear shoes (1,4,5), thereby negatively affecting their daily life.
