The impacts of the national centralized drug procurement policy on the use of policy-related antibiotic agents: the case of Shaanxi province, China

We aimed to explore the impact of the ’4 + 7’ policy on the use of policy-related oral antibiotics. Therefore, cefuroxime and its alternative antibiotics were chosen for study. Alternative agents refer to antibiotics that have a substitute relationship with cefuroxime in clinical use. The inclusion of alternative products mainly refers to a notice titled Reference Monitoring Scope of Substitutable Drugs of National Centralized Drug Procurement issued by the National Healthcare Security Administration [16]. In that document, 2 oral cephalosporins were recommended as partially replaceable agents to cefuroxime and 8 oral cephalosporins were recommended as alternative agents to some extent for monitoring. However, cefetamet pivoxil and cefditoren pivoxil are rarely used in public hospitals in Shaanxi. Hence, cefuroxime and 8 oral cephalosporins alternatives were included in this study.

The latter approach is a more popular strategy that has been employed by scientists. Another fairly new strategy described by Papp-Wallace and Bonomo, (2016) is the use of beta-lactam enhancers (BLEs), which describes a new class of antimicrobial drugs [16]. Although they are co-administered with beta-lactams, BLEs target a different PBP as compared to beta-lactams, thus ensuring bactericidal activity is achieved synergistically [30]. Zidebactam, is an example of a bicyclo-acyl hydrazide that functions as a BLE, and has successfully completed phase 2 clinical trials, and is set to enter phase 3 trials [31]. Another targeting strategy is the use of prodrugs as in the example of cefditoren pivoxil that is an orally absorbed prodrug hydrolyzed in the intestine to generate the active cephalosporin antibiotic, cefditoren [32].

In order to find an oral bactericidal antibiotic combination, we focussed our attention on cefditoren. This third generation cephalosporins has good activity against Gram positive cocci, because of its high affinity for penicillin binding protein-2 and 3, included, differently from other cephalosporins, penicillin binding protein-2x of Streptococcus pneumoniae. Penicillin binding protein-2x has affinities with penicillin binding protein-4 of E. faecalis and it might explain a certain activity of cefditoren against E. faecalis. Moreover, cefditoren MICs against E. faecalis are the lowest among other oral cephalosporins although MIC50 and MIC90 are still above 100 mg/L [5].
