Classification systems and ontologies are not foreign to the healthcare domain. They have evolved in healthcare alongside our ever-increasing ability to discover and distinguish disease and pathology as well as the ever-increasing number of procedures and services that may be deployed to address those diseases. Examples of widely recognized classification systems include the International Classification of Diseases (ICD), currently in its 10th edition, for terming and classifying diseases, with its sister ICD-PCS for procedure coding maintained by the World Health Organization.26 In the United States, Current Procedural Terminology (CPT) code sets maintained by the American Medical Association are principally used to report medical, surgical and diagnostic procedures using a coding classification system.

Medical coding is useful to life care planners and SLPs (Table 9.9) for documentation and billing purposes. Current Procedural Terminology, 4th Edition (CPT), is a systematic listing and coding of procedures and services performed by physicians, based upon the procedure being consistent with contemporary medical practice and being performed by many physicians in clinical practice in multiple locations. Each procedure is identified with a 5-digit CPT code. International Classification of Diseases-10-Clinical Modification (ICD-10-CM) is an indexing of medical information by disease and operations. V codes are codes within the ICD-10-CM classification system that may be used in any health care setting. V codes may be used as either a first listed (principal diagnosis code in the inpatient setting) or secondary code, depending on the circumstances of the encounter. V codes indicate a reason for an encounter and are not procedure codes. A corresponding procedure code must accompany a V code to describe the procedure performed. Readers should refer to the American Medical Association's bookstore website (www.amabook-store.com) for current manuals or software with up-to-date listings of all CPT, ICD-10 codes, and V codes. Table 9.8 lists the newest ICD-10-CM SLP codes in 2016. Table 9.9 lists all the speech-language pathology CPT codes as of time of publication. Table 9.10 offer example life care plan entries with costs. The Model Superbill for SLPs (Appendix 9.12) (ASHA, 2016) at the end of this chapter is another point of reference for SLP medical coding.

Also, in the US, Medicare has made advances to extend coverage to digital solutions like remote monitoring and telehealth—almost 20% of all new codes coming through the CPT process can be classified as having a digital health component (Nathanson, 2020). CPT (Current Procedural Terminology) codes are medical codes assigned to medical, surgical, and diagnostic services. They are set by the American Medical Association and used as a uniform language describing medical services and procedures between physicians, patients, and payers to streamline reporting.71 While some DTx may be eligible for reimbursement under these various codes, we have not yet seen specific CPT codes for DTx. This halts public coverage of such solutions and delays private coverage since commercial payers typically tend to follow and look to Medicare as an example for coverage decisions. DTx companies have been lobbying for a new senate bill that would specifically recognize prescription DTx as eligible for reimbursement, albeit limited to mental health indications. The “S.3532 Prescription Digital Therapeutics to Support Recovery Act” was submitted to the Senate Finance Committee in late March 2020.72 As of May 2021, we are yet to hear of any outcomes from the submission of this proposal, but with increased pressure from lobbyists, we are likely to see reform in the coming years.

Both the remedē® System and the Inspire® UAS Therapy are covered by insurance companies (Inspire Medical Systems, Inc 2015). The Current Procedural Terminology (CPT) codes are provided by the respective company to be used when implanting, activating, titrating, and managing the therapy. The CPT codes for activating and titrating the remedē® System in a sleep lab setting are 0434T for interrogation of the device with no setting changes, 0435T for interrogation of the device with setting changes, and 0436T for PSG and interrogation (CPT code 95810 is not to be used). The CPT codes for activating and titrating the Inspire® UAS therapy in a sleep lab setting are 95970 for interrogation of the device with no setting changes, 95974 for interrogation for the device with setting changes, and 95810-TC + 95974 for a PSG titration of the Inspire® UAS therapy. Since CPT code updates occur from time to time, periodic verification of suitable CPT codes should be requested from the corresponding manufacturer.

The majority of procedure codes derive from the Current Procedural Terminology (CPT) and Healthcare Common Procedure Coding System (HCPCS). CPT codes are developed, maintained, and are a registered trademark of the American Medical Association (AMA, 2012, WEB). HCPCS codes are developed and maintained by CMS and capture services not included in the CPT codes (CMS, 2012). Codes are predominately assigned by individuals trained to review medical records and code diagnoses and procedures. Veterans’ health records are predominately maintained by healthcare providers and retained in the VHA’s Computerized Patient Record System (CPRS) database. The process of coding requires healthcare providers to adequately document clinical encounters and for the trained coders to translate that documentation into available codes.

According to HCP, the communication difficulties with people with aphasia challenge the provision of assessments and therapy sessions and lead to negative feelings in HCP. HCP suggested that communication can be improved by increasing HCP knowledge and skills through CPT interventions, if attention is paid to making organizational changes and changing the roles of SLTs. Organizational changes include the provision of more time in the healthcare pathway and adapting healthcare information to the needs of people with aphasia. Physicians and managers should feel responsible for facilitating CPT for all HCP and evaluating the use of techniques in healthcare centres. The findings in this study point towards four specific roles that SLTs have to fulfil in order to guide HCP in using supportive conversation techniques.
