The group of perforating dermatoses includes elastosis perforans serpiginosa, perforating collagenosis, reactive perforating folliculitis. and acquired perforating dermatosis (syn. Kyrle's disease). Dermoscopy of the latter three entities is very similar and typically displays three concentric areas, the so-called “trizonal concentric” pattern, consisting of a central, roundish, brown-yellow structureless area surrounded by a white keratotic collarette and an outer erythematous halo, with or without dotted vessels (Figure 12b.16). A peripheral brown pigmentation has also been described in Kyrle's disease.46–48

Differential diagnosis: The most common considerations include AKs and Bowen’s disease. DSAP may mimic guttate psoriasis. Other less common diagnoses include seborrheic keratoses, stucco keratoses, elastosis perforans serpiginosa, and lichen planus. Epidermal nevus, linear psoriasis, and lichen striatus are included in the differential for LP.

Kyrle’s disease (KD) is a primary perforating dermatosis which includes also elastosis perforans serpiginosa, perforating folliculitis and reactive perforating collagenosis. The secondary form of perforating dermatosis is represented by acquired perforating collagenosis (1). The transepidermal elimination of altered skin components (keratins, collagen, or elastin) is the key pathogenetic feature of these disorders (2).
