Certain allergens in sensitized individuals cause a hypersensitivity pneumonitis also known as extrinsic allergic alveolitis (EAA). There are a number of subtypes of EAA including; farmers’ lung caused by Actinomycetes sp. in mouldy hay, bird-fanciers’ lung caused by proteins in bird droppings and cheese-workers’ lung caused by the fungus Penicillium sp. in cheese mould. Cryptogenic fibrosing alveolitis, as the name suggests, has no known cause.

This classification is hotly debated nowadays, particularly because it does not take into account the pattern of exposure and the progression of the disease. A group of experts proposed a new HP classification that differentiates two types [15], namely: type 1, where exposure is massive and intermittent (as in farmer’s lung disease) and the progression might preferentially be towards emphysema [13,16]; and type 2, where the exposure is permanent but less intense, (as in bird fanciers’ lung (BFL)) and might lead preferentially to pulmonary fibrosis.
